COVID-19 Waiver and Declaration

This waiver & declaration must be completed prior to visiting the Coppinwood Golf Club for the first time upon reopening. A verbal declaration will also be required on every visit to the Golf Club. If you have any questions, please email us pbussiere@coppinwood.com

The Coppinwood Golf Club appreciates your cooperation in following the COVID-19 protocols which have been put in place to protect members, staff and our community. These protocols have been established based on recommendations from the Province of Ontario Health and Safety Association Guidance documents and sport governing body recommendations.

The Coppinwood Golf Club is reopening based on the assurances members make to the Coppinwood Golf Club and to each other in this document and on each visit to the Coppinwood Golf Club. Accordingly, any false statements could result in the Coppinwood Golf Club being forced to deny member access. Further, members agree to observe and fully abide by all rules and protocols set out by the Golf Club.

As you are all aware, the virus can be transmitted by asymptomatic people and accordingly the statements made by members contained in this document cannot provide certainty that the virus will not be transmitted. The Coppinwood Golf Club is taking prudent steps to impose and enforce appropriate protocols to keep members safe, but there can be no assurance that the virus will not be contracted while at the Coppinwood Golf Club. This is a risk that each member must assess themselves when deciding to visit the Coppinwood Golf Club.

By completing and submitting this declaration you agree to the following.

Member agrees that their participation in Golf at the Coppinwood Golf Club is voluntary. Furthermore, the Member agrees to assume any risk associated with being infected with Covid-19 while being on the Coppinwood Golf Club property. The Member hereby releases the Coppinwood Golf Club from any and all claims of loss or damage that may arise during the Golf Season associated with Covid-19 only. Personal Well Being - The Member hereby warrants that he/she is healthy, is not suffering from and/or presenting with any Covid-19 symptoms and herby agrees to have their temperature taken upon arrival at the Coppinwood Golf Club and answer all required questions, truthfully. Right to Cancel - the Coppinwood Golf Club maintains the right to deny any Member the right to enter the facility if they are presenting with any symptoms of Covid-19 or has travelled to the Coppinwood Golf Club with a person who is not a member of their household. Applicable Law. This waiver agreement shall be governed by the laws of the Province of Ontario. Acknowledgment - The Member agrees that this waiver agreement shall remain in full force and effect during the 2021 Golf Season without modification, and that entry onto the the Coppinwood Golf Club Property is conditional upon agreeing to the terms of this agreement.

I hereby declare that I or anyone in my household has not experienced any cold or flu-like symptoms in the last 14 days (to include fever, cough, sore throat, respiratory illness, difficulty breathing). If I or anyone in my household experience any cold or flu-like symptoms after submitting this declaration, I will then not visit the club for a minimum period of 14 days after the cold or flu-like symptoms have ceased

I hereby declare that I will inform the the Coppinwood Golf Club's General Manager, PBussiere@coppinwood.com immediately if I or anyone in my household experiences any cold or flu-like symptoms, or has received a positive test for Covid-19

I hereby declare that I or any member of my household have not traveled to or had a lay-over in any country outside Canada in the past 14 days and have been advised to quarantine. If I or anyone in my household travel to any country outside Canada after submitting this declaration and are not exempt from quarantine, I will then not visit the Coppinwood Golf Club for a minimum period of 14 days after the date of return to Canada.

I have read and understand the below "THE GOLF CLUB'S COVID-19 MEMBER WAIVER & DECLARATION" as communicated by the Coppinwood Golf Club and accept and waive any right to Coppinwood Golf Club privileges should I not comply with the protocols.
