Participation Disclaimer & User Agreement

Please read, initial, and sign the following User Agreement before participating in all coaching programs, classes, workouts, and training sessions provided by FIERCE By Choice LLC, online and in person.

 

  • It is strongly recommended to consult your physician before beginning any exercise program to ensure you are in good physical condition. You should be in good physical condition and be able to participate in exercise before you begin any physical activity with FIERCE By Choice LLC.

  • You also agree that, to your knowledge, you have no limiting physical conditions or disability that would preclude an exercise program. If you are unsure of your condition, please consult your physician.

  • FIERCE By Choice LLC is not a licensed medical care provider and represents that it has no expertise in diagnosing, examining, or treating medical conditions of any kind, or in determining the effect of any specific exercise on a medical condition.

  • You should understand that when participating in any exercise or exercise program, there is the possibility of physical injury. If you engage in any exercise with FIERCE By Choice LLC, you agree that you do so at your own risk, are voluntarily participating in these activities, assume all risk of injury to yourself, and agree to release and discharge FIERCE By Choice LLC, from any and all claims or causes of action, known or unknown, arising out of FIERCE By Choice LLC, negligence.

 By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.
