MAD COOL FITNESS WELLNESS PROGRAMS

WAIVER & RELEASE

You understand that your participation in the MAD COOL FITNESS seminars, classes, programs, training sessions, and related activities may be hazardous to you or inherently dangerous and create risks of personal injury. You hereby expressly and specifically assume the risk of injury, harm or damages of whatever nature caused by your participation in the seminars, classes, programs, training sessions, and related activities.

You acknowledge and agree that, you are in good health and in sufficient physical condition to participate in the MAD COOL FITNESS seminars, classes, programs, training sessions, and related activities and that, if you have any concerns regarding you physical ability to participate, you should discuss them with our physician or other medical provider prior to participating in the seminars, classes, programs, training sessions, and related activities.

In addition, you hereby agree to release and forever discharge and hold harmless MAD COOL FITNESS, LLC, its members, managers, directors, employees and affiliates from any and all liability, obligations, claims, negligence, demands, and causes of action, of whatever kind of nature, either in law or equity, which may arise in any way from your participation to the MAD COOL FITNESS seminars, classes, programs, or training sessions and related activities, or your use of our training materials and or other materials or products, including, but not limited to any bodily or other injury, illness, death, or property damage.

You hereby grant permission to MAD COOL FITNESS, LLC its members, managers, directors, employees and affiliates to use or authorize others to use any photographs, motion pictures, recordings, or any other documentation of your participation in any MAD COOL FITNESS class, training seminar or related activities for any purpose, in their sole discretion. With this permission, you waive all personal rights and objections to any use of your name, personality, likeness, voice, or any interview given by you which may be used, displayed, performed, distributed, and otherwise exploited for any purpose whatsoever, including, without limitation, for use in the news media. You acknowledge that you will not be entitled to any payment for granting such permission and that you will not have the right to review or approve any usage.
