BICYCLE ACCIDENT WAIVER, AND INDEMNITY FORM RELEASE OF LIABILITY

I accept for use, the equipment listed on this form and accept full responsibility for the care of this equipment while it is in my possession. I agree to reimburse Alabama Coastal Amusements, LLC for the full retail value for any loss or damage other than reasonable wear and tear. I agree to return all rental equipment by the agreed time and in good condition to the agreed location to avoid any additional charges. I understand bicycling does invoke risk and I freely assume and understand protective gear such as a helmet is recommended but does not eliminate risk and may not reduce the risk of injury in the case of an accident.

I agree to release Alabama Coastal Amusements, LLC and the Gulf State Park from any and all responsibility of liability for injuries or damages to the user of this equipment listed on this form or to any other user of the equipment designated by the person listed below. I agree not to make a claim or suit against Alabama Coastal Amusements, LLC or the Gulf State Park for any mishaps or injury relating to the use of the rented equipment.

I accept the terms of the contract. This document constitutes the agreement between Alabama Coastal Amusements, LLC and the Gulf State Part and the undersigned. The rental equipment is accepted “as is”.

I have read this agreement and fully understand its contents. I understand this form is a release of Liability and the contract between Alabama Coastal Amusements, LLC, The Gulf State Park and myself and I sign it of my own free will.

Damage, loss, or theft of the bike will result in additional charges as follows:

Complete loss, theft, or destruction of E-Bike $1200

Complete loss, theft, or destruction of manual bike $500

Water damage to E-bike $1200

Damaged handle bars $50

Damaged headlight $20

Damaged taillight $25

Damaged battery $549

Damaged handlebar grips $15

Damaged pedals $20

Damaged seat $25

Burned out tires $40

Damaged child seat $40

Lost lock $10
