Flyers All-Starz 2021-22 contract

Hereby, I, athlete 18 years of age and over or parent  / legal guardian of the athlete, authorize the child hereinafter called ATHLETE, to retain the cheerleading and / or tumbling training services at the Flyers Cheer Gym located at 9233 rue Thimens, Pierrefonds, Qc H8Y1A0 for the season starting Sept 11, 2021 until May 31, 2022 or any other date agreed between the athlete and Flyers.

I recognize that the sport of cheerleeding involves risks which could cause injuries. I agree to inform Flyers All-Starz in writing, from the first training, of any medical and/or physical conditions which could affect the practice of this physical sport.   I release and hold harmless without limitation the owners, coaches, employees and volunteers of Flyers All-Starz from any responsibility in this regard, for any damage or injury and/or loss arising therefrom. I understand that the practice of this sport involves inherent risks and dangers. I certify that the ATHLETE is able to practice this sport. I also grant permission to a medical team or specialists chosen by Flyers to provide emergency care to the ATHLETE when/if necessary. I understand that I will be notified as soon as possible should such a situation arise. I believe the athlete is physically and emotionally capable of participating in this sport and is committed to following the rules and instructions of the coaches and the gym. In addition, the athlete undertakes to withdraw from the activities of Flyers All-Starz and to notify the coaches if he believes he observes conditions or situations which he finds potentially dangerous and/or if his physical or mental health is deteriorating.

I authorize  Flyers All-Starz Cheerleading to use the ATHLETE photos / videos on their website, social networks, in their brochure or any other advertising for purposes specific to Flyers All-Starz. I also agree that the name of the ATHLETE may appear on the Flyers All-Starz website and social media platforms.

I confirm that I have read the 2021-22 season information document and accept the terms thereof. By registering with Flyers All-Starz, the ATHLETE (or myself as an athlete aged 18 and over) undertakes to be present at ALL practices, activities, shows and competitions related to the team of which he/she is a member, according to the results of the rankings carried out by Flyers All-Starz management team and coaches. The trainings and / or competitions can be in person or virtual forms depending on the possibilities.
In addition, I agree to respect ALL Flyers rules at all times, otherwise, my position and role on the team may be changed and the possibility of removal from the team WITHOUT reimbursement could be considered. I understand and accept the possibility that a team includes more athletes than the maximum number allocated on the competition floor to ensure the performance of the team. I understand that full members of the team may not participate in certain competitions and that the level, category and division of the teams are subject to change without notice. Flyers reserves the right to swap athletes of teams as needed at any time during the season. Any sums of money or gifts won by Flyers teams in competition, remain the sole property of Flyers All-Starz and reserves the right of use at its own discretion.

I understand that Flyers Cheer Gym agrees to offer me monthly payment terms in order to spread my seasonal costs over a few months and that this is NOT a successive contract or a monthly subscription. I also agree to defray all costs related to these courses, and this, on the agreed dates, including non-refundable fees and services such as deposits, competitions, uniform, technical and training fees, and other additional fees/services.

This contract is an integral part of the Flyers All-Starz Montreal 2021-22 document which is always available in this link: https://s3.amazonaws.com/n1media1.files1.jamspiritsites.com/66471/122775015561257f8f70aab.pdf. I acknowledge having read, understood and accepted the following pages of the Flyers All-Starz season 2021-22 information document including: the price list detailed onpages 5 and 7 (prices may be subject to change without notice), the Flyers All-Starz regulations on page 12 as well as the cancellation policy detailed on page 15 and I declare myself satisfied with it. I understand that all training fees, additional fees and competition fees and all service fees offered to the athlete due upon registration (see below for payment options). I fully accept the Flyers All-Starz refund policy as set out in the 2021-22 season information document on page 15.

In addition, I agree to comply with these documents as long as I am a Flyers All-Starz athlete.

AUTHORIZATION OF AUTOMATIC PAYMENTS FOR FLYERS ALL-STARZ SEASON 2021-22

I, 18 years old + athlete or parent/legal guardian of a minor athlete, authorize Flyers All-Starz to automatically debit payment(s) of the 2021–22 season fee in full. The different payment agreements (1, 2, 4 or 7 payments) as well as the available payment methods (credit or debit card) are detailed in the section at the bottom of this document.

This authorization can be revoked ONLY by written notice from the signatory and must be received by Flyers no later than 24 hours after the call of your result by email at annulation@flyerscheerleading.net otherwise registration and planning payments will be made as scheduled. I understand that a "team creation" fee of $100 as mentioned in the 2021-22 information document, will be charged to me if I decide not to register for the 2021-22 season and / or if I leave at during the season.

The undersigned understands that this payment agreement constitutes his/her agreement for the registration of the above-mentioned athlete for the full 2021-22 season with Flyers All-Starz as well as his/her registration and recording of the monthly payments. This authorization confirms the athlete's place on his/her Flyers team. All season cancellations must be made in writing to the above-mentioned email. Please note that the cancellation will be made on the date of receipt of your email and in accordance with our refund policy (see the 2021-22 season information document on page 15 concerning applicable non refundable fees and services). Flyers All-Starz reserves the right to cancel on any registration.

In the event that the provisions of the banking information provided or credit card are insufficient, the signatory undertakes to assume the costs inherent in this situation. The undersigned certifies that the banking information mentioned is exact and complete. I agree that Flyers All-Starz will verify my below banking information by making a charge of $1 before obtaining my evaluation result. This charge of $1 will be reduced on the total amount of my 2021-22 season registration.

The payer agrees that the personal information in this authorization will be disclosed to Flyers All-Starz, provided that its disclosure is used only to make payments to Flyers All-Starz and/or to make the payment of a non paid such as a purchase of a package of private, semi-private, monthly season fees or any other late payment.
