Release Agreement

Please read carefully before signing

I agree to hold harmless and indemnify ​OKC Skateboard Lessons LLC​ and its owners, agents and employees for any loss or damage, including any that result from claims for personal injury, death, or property damage related to the activities provided by ​OKC Skateboard Lessons LLC​ (Releases). I further expressly agree that any legal action brought by me, on by behalf, or on behalf of my heirs, executors, personal representative, or assigns, against releases, wherein any damages are sought to be recovered from releases due to any loss, damage, injury, or any other event arising out of my participating in activities provided by ​OKC Skateboard Lessons LLC​, shall be deemed frivolous and groundless action pursuant to Oklahoma state law. 


I understand that there are inherent and other risks involved in the sport of skateboarding, that injuries including but not limited to the possibility of contusions, scrapes, bruises, concussion, or open fracture are common and ordinary occurrence of the sport, including the possibility of death, and freely and voluntarily assume those risks. 

 

I hereby release from any legal liability ​OKC Skateboard Lessons LLC​, and its owners, agents, and employees from any and all liability from damage and injury or death to myself or to any persons or property due to my participation in any and all activities provided by ​OKC Skateboard Lessons LLC ​and for any claim based on negligence, contract, claim or other legal theory, accepting myself, the full responsibility for any and all such damages or injury which may result. 


I hereby grant and authorize OKC Skateboard Lessons LLC the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. 


This authorization extends to all languages, media, formats and markets now known or later discovered.


This authorization shall continue indefinitely, unless I otherwise revoke this authorization in writing.


I waive the right to inspect or approve any finished product in which my likeness appears, including written or electronic copy.


I agree to this release without being compensated. I waive any right to royalties or other compensation arising or related to the use of the photograph. 


I understand and agree that these materials shall become the property of OKC Skateboard Lessons LLC and will not be returned.


I hereby hold harmless and release OKC Skateboard Lessons LLC from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.


Oklahoma Skateboard Training Facility Rules



1. Have Fun! Be chill, don't take yourself too seriously, and enjoy skating because life is too short not to!


2. Respect all OKC Skateboard Lessons Team Members, Other Skaters, and Parents: Treat everyone with courtesy and respect.


3. No Drugs or Alcohol or Weapons. 


4. No Food or Drinks in Skate Areas: Eating and drinking should be done in the lobby.


5. Stay Hydrated: Drink water regularly to stay refreshed and energized.


6. No Fighting or Roughhousing: Maintain a safe and peaceful environment.


7. No Excessive Noise: Avoid loud shouting or disruptive behavior.


8. Keep the Facility Clean and Respect Other's Property: Dispose of trash properly; leave areas tidy; don't touch other people's belongings; and don't forget your stuff before you leave!


9. Respect Personal Space and Wait Your Turn: Avoid crowding ramps or disrupting others skating. 


10. Keep Your Board With You: Don't leave your board in the skateable areas.


11. Wear Proper Safety Gear: Minors must wear helmets, no exceptions. We highly recommend wearing full pads on the vert ramp for all ages due to elevated injury risk.


12. Know Your Limits: Skate within your abilities and ask for help from a Team Member when you need it!


13. Help Fellow Skaters: Offer assistance and advice if someone needs help or guidance, or send them to an OKC Skateboard Lessons Team Member.


14. Report Safety Issues: Notify a Team Member of any issues or potential issues.


15. Don't Enter the Offices Without a Team Member.


16. Return borrowed boards and equipment to an OKC Skateboard Lessons Team Member.


17. Be on Time: Be punctual for scheduled lessons, camps, and sessions.


18. Limit Cellphone Use: Minimize distractions from personal devices.


19. We have the right to kick anyone out for any reason including none, and the right to ban anyone for any reason including none. Don't make us do this, please!


20. By entering the premises, you waive and release any claims you may have related to the use of recorded media of you at the facility, including, without limitation, any right to inspect or approve the photo, video or audio recording of you, any claims for invasion of privacy, violation of the right of publicity, defamation, and copyright infringement or for any fees for use of such record media.

You understand that all photography, filming and/or recording will be done in reliance on this consent. If you do not agree to the foregoing, please do not enter the premises.

 

I have read this agreement, the facility rules, the release of liability form and fully understand its contents.

 

I am aware that this is a release of liability and a contract between OKC Skateboard Lessons LLC and myself and I sign it of my own free will.
