Below is our release form. 

As a participant in Prenatal Yoga/Parent & Me Yoga at The Family Room, I agree to the following: 

I acknowledge that I have consulted with my doctor or midwife prior to beginning this yoga series. I fully understand that I am voluntarily participating in this program.  


I voluntarily assume the risks associated with prenatal and postpartum exercise of any kind. I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which I or my baby might incur as a result of participating in the program. I understand that students are encouraged to practice at their own pace, and only engage in postures/poses that feel right for their bodies.


In further consideration of being permitted to participate in the yoga classes, I knowingly, voluntarily, and expressly waive any claim I may have against the instructor, the owner, or the leaseholder of the building for injuries or damages that I may sustain as a result of participating in classes or workshops held at The Family Room. 


I have read and fully understand and agree to the above terms of this Liability Waiver Agreement. I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law in the State of California.

I hereby represent that I have carefully read, understand and agree to the contents of this Liability Waiver Agreement and consent and sign the same voluntarily and of my own free will.
