Liability Waiver


I, acting as myself or the parent or guardian of the participant listed, acknowledge that participation in athletic events necessarily involves risk of physical injury. I understand that my child will be participating in strenuous physical activity, and I am entirely responsible for ensuring that my child is physically able to participate, and my child has no known medical conditions that would restrict participation.

In consideration for accepting the participation in the OpenGym LLC (OpenGym) event(s) for the Participant noted, I hereby release, discharge and hold harmless the OpenGym staff, volunteers and owners from any claims arising out of, or relating to any physical injury that may result to the participant noted, while participating in a OpenGym event, including any physical injury caused by the negligence of any OpenGym staff, volunteer, or owner while performing their duties during the event for OpenGym

I acknowledge that I understand that at OpenGym LLC all areas of the gym facility are recorded 24/7. As part of the offerings at OpenGym cameras are used for security, loss preventions as well as to track analytics for the participants in the gym. I hereby authorize OpenGym LLC to use my Image, Likeness and name, still photography, data, video or live streaming. OpenGym is authorized to use these images to promote OpenGym  

I understand that I, or my noted, designated emergency contact will be contacted in the event my child is deemed to need medical attention. In the event that I, or my emergency contact, cannot be reached in an EMERGENCY, I hereby give permission to the attending (IF attended) OpenGym Staff representatives to secure proper treatment for my child.

By completing the purchase this session at OpenGym, you are agreeing to the terms of this waiver.
