Patch Wood Farm Dog Boarding Agreement

 

 

  1.  Owner agrees to pay the rate for boarding of $45 per day, $5 discount for Patch Wood dogs. Boarding rates are calculated per calendar day.

  2. Pick up and drop off appointments are available by appointment only Monday through Friday 9am or 5pm, Saturday and Sunday 9am only. Should you be more than 10 minutes late for your scheduled appointment, a late fee of $15 per half hour will be charged, and we will need to reschedule depending on circumstances to the next available appointment.

  3. A 2 day deposit is due to hold your spot. The deposit is non refundable unless canceled 1 week prior to scheduled drop off. You are responsible for full charges of all booked days, even if the dog gets picked up early. 
  4. Should your dog be an intact male or come into heat during their stay, there will be a $10 surcharge per day.  
  5. The Owner further agrees to pay for all costs and charges for any special services requested. Owner agrees that the dog shall not leave until all charges for boarding, grooming, medicine, veterinary services, play time, etc. are paid for by Owner.
  6. Dogs are required to be UTD on Rabies and Distemper, as well as clear of fleas and any other diseases. Should your dog have fleas, we will treat them accordingly and add it to the bill.
  7. Veterinary Information and Emergency Care. If Owner's dog requires emergency medical treatment, Patch Wood Farm will attempt to contact Owner with provided phone numbers, but in the event Owner cannot be reached after reasonable efforts, Patch Wood Farm shall exercise its reasonable judgment in providing medical care for Owner's dog. IN THE EVENT THAT PATCH WOOD FARM CANNOT CONTACT OWNER AFTER REASONABLE EFFORTS, PATCH WOOD FARM MAY, IN ITS SOLE AND ABSOLUTE DISCRETION, CHOOSE  TO ARRANGE FOR APPROPRIATE MEDICAL TREATMENT FOR THE DOG .  

     
  8. Owner shall be responsible to pay all costs related to such emergency care should it be provided, and Patch Wood Farm is hereby authorized as Owner's agent to arrange for billing to be provided to Owner. Owner shall at all times maintain emergency contact numbers with Patch Wood Farm and shall ensure they are updated.
  9. All dogs are boarded by PWF without liability on our part for the loss or damage, from disease, death, running away, theft, fire, injury to persons or other animals or property by said dog, fence climbing or other unavoidable causes, due to diligence and normal care and caution having been exercised.
  10. Owner agrees to have disclosed all behavioral, physical or medical issues the dog may have.
  11. By signing this contract and leaving your dog with PWF, Owner certifies to the accuracy of all information provided about the dog, that they are current on all vaccines required by PWF (rabies & distemper), and that the dog has not been exposed to rabies, distemper or any other illness within a 30 day period prior to boarding. Owner is the legal owner of this dog, free and clear of all liens and encumbrances and assumes all financial liability for all payments called for in this agreement.
  12. This contract contains the entire agreement between the Owner and PWF. All terms and conditions of the contract shall be binding on The heirs, administrators, personal representatives and assignee of the Owner and PWF. 
  13. Owner acknowledges inherent risks of dogs playing in a group setting. Should your dog not get along with other dogs, other arrangements may be made by Patch Wood Farm and owner.  
  14. This contract applies to this stay at PWF as well as all subsequent stays.
