RELEASE OF LIABILITY AND WAIVER OF ALL CLAIMS BY PERSONS ENTERING THIS FACILITY

I, individually (or as a parent or guardian of a minor child under the age of 19 participating) understand, acknowledge, and agree that ice skating is an inherently dangerous and physically demanding activity. Participating skater assumes full responsibility for any risk of injury, property damage, or death while participating in activities at this establishment.

In consideration of the City of Tuscaloosa permitting the undersigned or his/her minor child to engage in the aforesaid ice skating activity, the undersigned does, for himself/herself, his/her heirs, assigns, executors and administrators, remise, release and forever discharge the City of Tuscaloosa, a Municipal Corporation, its officers, agents, and/or employees, event sponsors , its officers, agents and/or employees, event volunteers of and from all manner of action and actions, suits, and sums of money, dues, claims, or demands, whatsoever, which arise out of or in any manner grow out of, property damage sustained by the undersigned or his/her minor child or injuries or death sustained by the undersigned or his /her minor child , including injuries known or unknown, by reason of being permitted to participate in the aforesaid activity in the City of Tuscaloosa.

The undersigned (or minor child participant) agrees that they may be photographed or filmed during the activity and consents to the City using the participant's picture and likeness contained in any photographs or film for publishing and for promoting any future events to be conducted by the City without any fee or other compensation.

The undersigned further agrees that he/she (or my minor child participant) will abide by the “Code of Skater Responsibility”, as posted within the ice rink area, for the use of the ice skating rink and will follow the instructions of the operating staff of the rink. Failure to follow these codes of conduct may result in the termination of Participant’s use of the ice skating rink, the rental ice skates, and the surrounding areas without refund.

THE UNDERSIGNED has read this Release and fully understands the same.
