2024 Chicago Bears Training Camp and Bears Care Back to School Fair
Waiver and Release of Liability

In return for and as an inducement and material condition precedent to The Chicago Bears Football Club, Inc. (“Chicago Bears”) and Bears Care (“Bears Care”) permitting the individual(s) listed below (hereinafter individually and collectively referred to as the “Participant”) to participate in any and all activities at 2024 Chicago Bears Training Camp and Bears Care Back to School Fair at Halas Hall located at 1920 Football Drive, Lake Forest, IL 60045 (the “Event”) (including, but not limited to (a) virtual reality; (b) 360 photo spinner; (c) Mini Monsters skills and drills; (d) “Athletic Trainer Experience”; (e) “Pad Experience”; (f) football toss; (g) forty-yard dash; (h) agility drill and field goal kick; (i) haircuts; and (j) face painting (collectively referred to as the “Activity”)), the undersigned individually and on behalf of the Participant and the Participant’s respective, heirs, executors, administrators, estate, family, and assignees, agrees to and does hereby irrevocably release and agree not to sue the Chicago Bears, Bears Care, Event sponsors, the National Football League, media partners, vendors, contractors, and/or each of their respective subsidiary companies, officers, shareholders, directors, affiliates, agents, employees (hereinafter individually and collectively referred to as the “Released Parties” or “Releasees”) from all present and future suits, claims, demands, losses, costs, charges, expenses, and/or liabilities from and/or for property damage, personal injury or wrongful death (even if caused by the ordinary negligence of the Released Parties listed above), in any way arising out of or related to the Participant’s participation in the Event and/or any Activities. I understand that participation in the Event involves certain risks, including, but not limited to, serious injury or death, and any exposure to our transmission of COVID-19 arising out of the Event. I am voluntarily participating in the Event with the knowledge of the danger involved and agree to accept all risks of participation. I hereby agree to indemnify, hold harmless, and release from liability the Released Parties, individually and collectively, form any and all claims arising out of participation of any Participant (including myself) in the Activity and/or Event, including, without limitation, any liability for adverse reactions to any food or beverage(s) consumed or that Participant may come in contact with while at the Event or any items or products featured or distributed at or in connection with the Event, as applicable; and further that, the Released Parties are not responsible for any injury, illness or loss, damage, or destruction of property from any such cause and make no representations or extend any warranty with respect to any such item(s) or product(s). 

Additionally, Participant grants irrevocable permission for the Released Parties and their respective sponsors, licensees, advertisers, broadcasters, designees, partners and agents to use, publish, distribute and/or modify Participant’s image, likeness, voice, actions, and statements, in any medium, including without limitation, audio, video, or images of the Event for any purpose (including commercial purposes) without further authorization or compensation, and Participant waives all claims and potential claims relating to such use, unless prohibited by law.

Participant understands that this document is intended to be a broad and inclusive as permitted by the laws of the State of Illinois and agrees that if any portion of this Agreement is invalid, the remainder will continue in full legal force and effect. Participant also agree to abide by the decisions of the Released Parties with regards to age, height and weight limitations of all attractions.
