THIS FORM MUST BE SIGNED BY THE MINOR’S LEGAL GUARDIAN AND TURNED INTO CAMP VICTORY BEFORE MINOR MAY ATTEND ANY EVENTS AT CAMP VICTORY. IF NOT SIGNED, PARTICIPANT WILL NOT BE ALLOWED TO ATTEND CAMP VICTORY. 

As the parent or legal guardian of the said minor, I acknowledge that the camp and all related activities are offered by Camp Victory and authorize Camp Victory to use photos taken of minor for promotional and recordkeeping purposes. I also authorize said minor to attend Camp Victory and to engage in all camp activities, including water sports, horseback riding, outdoor activities, and strenuous activities. I authorize administration of medical treatment including the administration of over-the-counter medications deemed necessary by Camp Victory and agree to release and indemnify Camp Victory against all liability and costs for treatment.

Regarding COVID-19 or any other virus that is spread through person-to-person contact, I acknowledge that Camp Victory cannot guarantee that my child and/or I will not become infected whether or not due to attendance at Camp Victory’s Summer Camp. I voluntarily assume the risk that my child(ren) and I fully understand as a family and choose to attend Camp Victory.

By signing this waiver, I, individually and on behalf of my heirs, children, estate, executors, administrators, assigns, and all members of my family, voluntarily assume all risks associated with participation in any event held at Camp Victory and agree to release, indemnify, hold harmless, and covenant not to sue Camp Victory, Victory Christian Center, Inc., and their agents, representatives, parents/subsidiaries, volunteers, directors, and employees or contractors from any and all claims, losses, damages, fees/costs/expenses, or liability for harm to said minor, or minor’s personal property, resulting directly or indirectly from minor’s attendance at Camp and/or participation in any Camp-related activities – even if Camp is deemed negligent.
