NY Tattoo Removal Membership enrollment
and consent for automatic payment withdrawal
I understand that I am signed up for a monthly subscription for laser tattoo removal treatments which include 1 laser tattoo removal treatment every 60 days on the area size I have selected.
I give permission for my credit card on file to be charged every month for the amount of the treatment size I have choose. I also give permission for my card to be charged a 24 hour $25 cancellation fee if I should miss my appointment or not cancel/reschedule outside of 24 hours before my appointment. I can cancel anytime after the 6th month. If I do not use a treatment one month, it will roll over to the next month, or can be used as credit for other services so the value is never lost.
In the event my card declines for a monthly payment I agree to provide a new form of payment and understand I am responsible for being charged for at least 8 payments. Any declined payment will be charged a $9.95 fee
Membership Cancellation Policy
Cancellations must be submitted in writing by email to laserluxury@gmail.com.
To avoid being charged for the upcoming month, your written cancellation request must be received at least five (5) days prior to the 1st of the month in which you wish to cancel.
Cancellations received after this deadline will take effect the following month.
