If your child is participating in Private Instruction with a GCC staff member, please complete the Private Instruction Waiver Form! 

I hereby authorize and consent for my student to receive private lessons from GCC staff members. The signed waiver expires 1 year after signature date. Each year a new private lesson waiver will need to be updated. 

I understand that the following are the guidelines for private lessons: 

1. All lessons must follow the one-on-one policy. There are NO one-on-one privates allowed at GCC. Each private at minimum will consist of the coach, student(s), and a second adult.  The second adult may be another GCC employee or the parent/guardian of the student. 

2. A parent/guardian can observe each private lesson.

3. Each private lesson must take place inside the open space of the gym where viewing is optimal. 

4. I can withdraw by consent for private lessons at any time.
