Photo & Video Consent and Release Form

I hereby grant and authorize my provider permission to take, edit, alter, copy, exhibit, publish, and distribute any photographs or videos taken of me for use in legally permitted promotional materials. These may include, but are not limited to: social media, websites, brochures, flyers, posters, advertisements, newsletters, and other digital or print communications—without payment or any other compensation.

This authorization applies in all media formats and markets now known or later developed and shall remain in effect indefinitely unless I revoke it in writing.

I understand that these materials will become the property of my provider and will not be returned.

I release and hold harmless my provider from any and all claims, demands, and causes of action that may arise from the use of these images or recordings.

I confirm that I am at least 18 years of age and legally competent to sign this release. I have read this authorization and fully understand its contents and implications.
