Westgate Indoor Skatepark

I, the undersigned, hereby acknowledge and agree to the following:

1. Voluntary Participation

I understand that participation in skateboarding and related activities at Westgate Indoor Skatepark involves inherent risks, including but not limited to falls, collisions, equipment failure, or other accidents that may result in injury, permanent disability, or even death.

2. Assumption of Risk

I freely and voluntarily assume all risks associated with participation in activities at Westgate Indoor Skatepark whether known or unknown, including the risk of injury caused by the negligence of WestgateIndoorSkatepark, its staff, volunteers, or other participants.

3. Release of Liability

I hereby release, waive, discharge, and hold harmless Westgate Indoor Skatepark, its owners, officers, employees, volunteers, sponsors, and agents from any and all liability for any injury, loss, or damage to person or property, including death, arising out of or related to my use of the facility.

4. Medical Treatment

I authorize Westgate Indoor Skatepark obtain necessary medical treatment for me in the event of injury, accident, or illness, and I agree to be responsible for any and all medical expenses.

5. Protective Gear Requirement

I acknowledge that wearing a helmet and other protective gear is strongly recommended and may be required. I accept full responsibility for using or not using such equipment and understand the risks of not doing so.

6. Photography & Video

I grant permission for Westgate Indoor Skatepark use any photographs or videos taken of me for promotional or commercial purposes without compensation.

7. Minors

If the participant is under 18 years of age, a parent or legal guardian must also sign this form.
