AERIAL KIDS ACADEMY (owned and operated by Aerial Arts Limited) prioritizes safety above all else in all its activities. We value the well-being of our participants as our top priority. While we are dedicated to minimizing risks, it is important for parents and accompanying guests to acknowledge the inherent risk of injury associated with recreational activities. Aerial Arts Academy is committed to ongoing efforts to mitigate these risks and emphasizes the importance of adhering to safety protocols and instructions meticulously crafted to safeguard the well-being of all participants. RELEASE OF LIABILITY WAIVER: In consideration of Aerial Kids Academy accepting myself, my child, and any accompanying guests into participation and/or training in aerial acrobatics & gymnastics activities, an activity which I hereby acknowledge involves a greater than normal risk of injury, I agree, for myself, or as my child's parent/guardian, to assume responsibility for all risks, costs, or losses sustained by me, my child, or my child's family and any accompanying guests in connection with participation in aerial acrobatics & gymnastics activities classes, programs, camps, lessons, meets, birthday parties, open gyms, field trips, or any other activities connected with Aerial Kids Academy. I give my permission to Aerial Kids Academy and/or the appropriate medical facility to take whatever emergency (first aid, disaster evacuation, etc.) measures as judged necessary for the care and protection of me, my child, any accompanying guests, while under the supervision of Aerial Kids Academy. In case of an emergency, I understand that I or my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the local emergency resources deem it necessary. Transportation will be at my own expense. It is understood that in some medical situations, the staff will need to contact the local emergency resource before the parent, physician, and/or others acting on behalf of the parent or family can be reached. Furthermore, I hereby release, agree to hold harmless, and indemnify Aerial Kids Academy employees, owners, or volunteers from any claims, losses, or expenses incurred on behalf of me, my child, any accompanying guests, or my child's family. CONSENT TO PHOTOGRAPH AND MEDIA RELEASE: I understand that my child's or any accompanying guests' photograph or video may be taken during the course of class instruction, during a special event at Aerial Kids Academy or at a function sanctioned by Aerial Kids Academy. I hereby grant permission to Aerial Kids Academy to use my child's or any accompanying guests' photograph or likeness in any publicity or promotional publications (e.g., website, newspaper ads, social media, newsletters, programs, brochures, public broadcast).
