Islamorada, Village of Islands Parks and Recreation Department Release, Waiver and Indemnification Agreement Activity: All Adult Sports, Programs and Activities at facilities owned by Islamorada, Village of Islands. I, the undersigned, agree that Islamorada, Village of Islands (the “Village”) and its officers, agents and employees shall not be held liable for injuries, death or other loss which may occur as a result of my participation in the above described activity, and that the undersigned voluntarily assumes the risk of any loss, injury or death to person or loss or damage to property, which in any way arises out of participation in said activity. Further, the undersigned WAIVES ANY CLAIM against the Village and its officers, agents and employees arising from loss, injury, death or damage and does COVENANT NOT TO SUE the Village and its officers, agents and employees. Further, the undersigned agrees to RELEASE, INDEMNIFY, DEFEND AND HOLD HARMLESS the Village and its officers, agents and employees from any and all claims, actions, demands, rights, judgments liability or expenses arising from or by reason of any and all know or unknown damages, claims or actions arising from participation in the above-described activity. This indemnification and hold harmless shall continue notwithstanding any negligence or comparative negligence on the part of the Village relating to such loss, injury, or damage. I hereby give permission for the Village and its officers, agents and employees to call my physician and or to arrange for transportation to a hospital in the event of any injury, although I understand that the Village and its officers, agents and employees assume no responsibility to do so. Participant also acknowledges and understands that the Village and its contractors, partners and/or sponsors may use photographs, video or film for educational, informational or promotional purposes, and Participant hereby grants the Village and its contractors, partners and sponsors permission to use images of Participant or Participant’s likeness for any purpose with no compensation or liability. I hereby agree that this Release form shall be binding on my heirs, successors and assigns. The undersigned has fully read, understood and agrees to each term contained in this Release, Waiver and Indemnification Agreement.

Adult Sports Code of Conduct A. The manager/captain of each team shall be responsible for seeing that each member of his/her team is familiar with the following rules of conduct and will also be expected to see that all members of each team abide by this code during the season. All participants are required to sign this pledge to be allowed to participate in league play. ALL PARTICIPANTS ARE REQUIRED TO ABIDE BY THESE RULES OF CONDUCT. - No player shall refuse to abide by an official’s decision, threaten an official, use profanity, or at any time lay a hand upon, push, shove or strike an Official.Penalty: The official shall eject the player from the game and report the incident to Founders Park Management (FPM). The player shall be automatically suspended until final disposition by the FPM. Suspension may be permanent, at FPM discretion, depending on the nature of the violation.
- No player shall demonstrate objection to an official’s decision by throwing any object that an official judges to be dangerous. Any other unsportsmanlike behavio r may result in suspension at the discretion of FPM. Penalty: Same as Rule #1
- No player shall use unnecessary rough tactics against the body and person of another player with the intention of causing injury; nor shall any player threaten strike or push any other player, spectator, or scorer. Penalty: Same as Rule #1
- No player or team member may be intoxicated immediately prior or during a game. No intoxicating substances shall be allowed in the Park including the playing court or field. Penalty: All intoxicated players will be ejected from the Park immediately and the game shall result in immediate forfeit. Violation of this rule will result in players and/or team being expelled from the league permanently.
- FPM shall have the power and full discretion of imposing penalties on all violations of the players’ code of conduct. For any violations not included in the said code, the penalty shall be at the discretion of FPM.
- If a player is ejected from a game, he or she is required to vacate Founders Park immediately.
