I, being the parent / legal guardian of the named athlete or the athlete if 18yrs+ consent that she/he may participate in cheerleading. I recognise the risks involved when taking part in any activity involving height and motion, including cheerleading. I accept that Team England, its staff and coaches are not responsible for determining whether the athlete is fit to take part in cheerleading. I confirm that the athlete is physically fit and healthy and I will advise Team England of any change. I declare that I consider the athlete capable of taking part in cheerleading.

In signing this consent form I declare that I am aware of the element of risk involved and while I accept that the coaches will take precautions to prevent accidents, I understand that they will not be held responsible for accidental loss, damage or injury to the athlete unless negligence can be proved.

I consent and authorise Team England and its staff and coaches to give the immediate necessary authority on my behalf for any medical treatment recommended by competent medical authorities in the case of emergency at any authorised event the athlete is participating in or attending. I further understand that every effort will be made to contact me. I will not hold Team England legally or financially responsible for any claims arising from consent and medical treatment for the athlete named.

 I am aware that photographs and video footage may be taken during authorised events for coaching and promotional purposes. I give permission for the athlete to be photographed or video-taped during any authorised event, and give permission for such photographs and video to be used for the promotion of Team England activities and publicity, and the sport of cheerleading. I waive the right to approve any photographs or videos and understand that there will not be any compensation for the use of these photographs or videos.

I hereby agree with the above, and have read and fully understand these conditions and by signing, agree to accept them.


Team England Code of Conduct & Commitment

 

If successful in gaining a place on Team England my athlete/I agree to the following:

1.    I accept the invitation to join and hereby commit myself to the Team England team for the 2025 ICU Cheerleading World Championships and all training leading up to this event.

2.    I understand that this is a full COMMITMENT and that the team is depending on me to fulfil my responsibilities as a team member. I will attend ALL TRAINING SESSIONS. Failure to do so will result in loss of position within the team. Should a serious scheduling conflict arise, I commit to providing my coaches with as much advance warning of absence as possible. If a training session is missed I understand I may not be allowed to compete.

3.    I understand that representing my country is a privilege and will not use inappropriate language or participate in inappropriate behaviour. I will not participate in illegal consumption of tobacco, alcohol or drugs. I understand that all of my actions both inside and outside the gym are a reflection of Team England and should model good behaviour. I will remember that I am a representative of Team England and must conduct myself in an appropriate manner at all times. This includes but is not limited to face-to-face, telephone, text, email and social networking sites, specifically Facebook and twitter.                                   

4.    I will come to practice with a positive ATTITUDE, and I will follow the instructions given by all Team England coaches and will work hard to achieve goals set by these coaches.

5.    I will display good sportsmanship at training and performances, and not disrespect any team member, coach, parent or competitor from Team England or any other team for any reason at any time.

6.  I understand that being given a place on the team does not guarantee me a space on the floor at the ICU Worlds in 2025. I will respect that the decision of the coaches is final. 

7.    I understand that all material, music, and routines are the property of Team England and are not to be used for any other purposes including allstar squads, talent shows, social networking sites, personal web pages and internet video sites.

8.    I understand that I will be responsible for organising my own travel to all team England training sessions. I will ensure that I am at all scheduled Team England practises and events on time without fail.

9.    I understand that breaking any of the above may result in suspension or dismissal from Team England.
