SECTION 1 - PURPOSE Spokane Valley Junior Soccer Association (SpVJSA) promotes an atmosphere of harmony and understanding at all facilities, programs and events and requests participants, spectators and staff to follow a common set of rules for courteous behavior. All participants enter the roster and agree to the athletics code of conduct. Upon signing, participants acknowledge they have read and agree with the athletics code of conduct, posted rules; and acknowledge the disciplinary measures in the event of a violation. SECTION 2 - CODE OF CONDUCT - Respect the rights and privileges of all persons at all times.
- Comply with requests from officials, staff and facility supervisors.
- Comply with any rules and guidelines of the program, and the host facility.
- Refrain from conduct that disrupts or obstructs any program or event. This includes rudeness to others an argumentative behavior.
- Refrain from lewd, obscene or indecent conduct or expression, including profanity, harassment, discrimination, threats, offensive remarks or gestures or racial, ethnic or sexual slurs.
- Refrain from any action which in the judgment of staff, facility supervisor or official, constitutes an attempt to inflict, or the actual infliction of, or injury to, other participants and/or staff.
- Destruction or damage to a SpVJSA facilities or one of its facility partners is not acceptable.
- Theft of any SpVJSA, partnering facility, or participant’s property is not acceptable. Anything found at the facility that does not belong to you should be left in place.
- Alcohol, tobacco and illegal drugs are not permitted at any facility.
- Possession or any use of a weapon or explosive devices is not allowed.
- Children must be supervised by a non-participating adult.
- In case of emergency, dial 911.
SECTION 3 - ENFORCEMENT If a violation occurs, SpVJSA Parks & Recreation staff will review event(s) with the participants and program staff. Violations of the code of conduct may result in one or more of the following disciplinary measures: - Verbal warning.
- Ejection of the current game/event. Officials and/or facility supervisors have authority to direct any person in violation of this code of conduct to immediately remove themselves from the premises.
- $50 Re-entry fee before next game.
- Forfeiture of game(s).
- Suspension from future games/events for a period of time determined by the recreation staff based on circumstances and severity of the incident.
- A permanent expulsion from all programs.
- Prosecution/reporting of illegal activity *not subject to appeals process.
- Participants determined to be in violation of the Code of Conduct and assessed disciplinary measures have the right to appeal that action.
- To appeal, one must complete the County Appeal Application Form and submit to the SpVJSA Parks and Recreation Department within five (5) business days of receipt of the official written Notice of Disciplinary Action along with an Appeal Application Fee of $250.00 and Deposit of $250.00, in accordance with the procedure set forth therein.
IN CONSIDERATION of being permitted to participate in any way in this PROGRAM &/OR EVENTS: - HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUBMIT CLAIMS OR FILE LAWSUITS against the Spokane Valley Junior Soccer Association (SpVJSA) its officers, employees, agents, local school districts and contractors FROM ALL LIABILITY TO THE EMPLOYEE/VOLUNTEER his personal representatives, legal guardians, assigns, heirs, and next of kin FOR ANY AND ALL LOSS OR DAMAGE, AND ANY CLAIM OR DEMANDS THEREFORE ON ACCOUNT OF INJURY TO THE ARISING OUT OF OR RELATED TO THE EVENT(S), EXCEPT THOSE SOLELY CAUSED BY THE NEGLIGENCE OF THE SpVJSA.
- HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the SpVJSA FROM ANY LOSS, LIABILITY, DAMAGE, OR COST they incur arising out of or related to the PROGRAMS &/OR EVENT(S) except those solely caused by SpVJSA's sole negligence.
- HEREBY ASSUMES FULL RESPONSIBILITY FOR ANY RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE arising out of or related to the PROGRAMS &/OR EVENT(S) except those caused by SpVJSA's sole negligence
- HEREBY acknowledge that THE ACTIVITIES OF THE PROGRAM &/OR EVENT(S) involve the potential risk of injury and/or death and/or property damage.
- HEREBY states that I am 18 years of age or older.
- Any player participating in an SpVJSA League or Tournament game without properly completing the roster form and having it submitted to the SpVJSA office. Player voluntarily assumes the risk of any and all injuries sustained and/or approximately caused by his willful and unauthorized participation in the recreation program and SpVJSA owes no duty to that player. In addition, any player participating without first properly completing the roster form and properly serving it to the SpVJSA will be suspended for remaining games, along with the Coach/Manager and the present game the player is participating in shall be an automatic loss by forfeit.
- I understand that sports can and should be a setting in which personal character, integrity, respect, responsibility and fairness are displayed. I will act in a way that is a credit to myself and my team to ensure that sports are a constructive social force. I have read, understand and agree to abide by the Athletics Code of Conduct and understand that there may be disciplinary measures that will be enforced if I choose to not abide by the rules of the game or the Athletics Code of Conduct. I, on behalf of my team and myself, in consideration of SpVJSA Parks and Recreation to facilitate a dispute resolution, hereby waive my legal rights to file suit in a court of law and agree to the terms, conditions and decisions of the SpVJSA Appeal Process as outlined herein.
- By providing my name, address, phone number and email address below I authorize SpVJSA to use the provided contact information to inform me of program updates and current department news through local mail, internet newsletters, text messages and phone calls. I agree that pictures taken during program hours may be used for promotional purposes.
- Player e is fully aware of risks and hazards connected with the activity, including potential exposure to the COVID-19 virus and hereby agrees to comply with all Federal, State, and Spokane Regional Health District requirements. Current state guidance can be found here
I HAVE READ THIS ATHLETICS CODE OF CONDUCT, RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL LEGAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. |
