In consideration of being permitted to participate in the [Name of Your Event] 5K Run/Walk (the "Event"), I, the undersigned participant, for myself, my heirs, executors, administrators, legal representatives, successors, and assigns, hereby acknowledge, understand, and agree to the following:

1. Assumption of Risk: I understand that participation in a 5K run/walk is a potentially hazardous activity. I knowingly and voluntarily assume all risks associated with my participation, including, but not limited to, falls, contact with other participants, the effects of the weather (including heat, humidity, and cold), traffic and road conditions, all such risks being known and appreciated by me.

2. Physical Condition: I certify that I am in good physical condition and am physically capable of participating in this Event. I have not been advised by a medical professional to not participate in such an activity. I understand that it is my responsibility to seek medical advice if I have any concerns about my physical ability to participate.

3. Release of Liability: I hereby release, waive, discharge, and covenant not to sue [Name of Event Organizer], [Name of Sponsoring Organization(s)], the event venue, the host city, and their respective directors, officers, employees, volunteers, agents, and representatives (collectively, the "Releasees") from any and all liability, claims, demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise.

4. Indemnification: I agree to indemnify and hold harmless the Releasees from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, brought as a result of my involvement in the Event.

5. Photo/Video Release: I grant full permission to the Releasees to use any photographs, videos, or other recordings of me for legitimate purposes, including event promotion, without any compensation to me.

6. Rules and Regulations: I agree to abide by all the rules and regulations of the Event and to follow all instructions of the Event staff and volunteers. I understand that the Event officials have the right to disqualify me from the race at any time for any reason they deem appropriate.

7. Voluntary Participation: I have read and understood this waiver and I am voluntarily signing this agreement. I understand that by signing this document, I am giving up substantial rights, including my right to sue.
