Pet Waiver & Dog Park Liability Release Form

(Day guests are not permitted entry with animals unless they are verified service animals )

Please read this document carefully. By signing this agreement, you acknowledge the inherent risks associated with off-leash dog parks and natural water elements and agree to waive certain legal rights.

1. New York State Law Acknowledgement

Under New York Agriculture and Markets Law § 123, a dog owner or lawful custodian is held strictly liable for all medical and veterinary costs resulting from any injury caused by their dog to a person, companion animal, or domestic animal.

  • By signing below, you agree that you are the legal owner or designated handler of the dog(s) entering the park, and you accept full financial and legal responsibility for their actions.
  • You acknowledge that Sleepy Hollow Lake Campground provides this off-leash space as a recreational amenity but does not supervise the area, evaluate individual dogs, or assume any liability for incidents occurring on the premises.

2. CRITICAL: Pond, Wildlife, and Water Safety Warning

The dog park area includes access to/proximity to the pond located within the former petting zoo grounds. Dogs are strictly prohibited from entering, swimming in, or drinking from the pond water.

Notice of Inherent Natural Hazards: This pond is a natural body of water and presents severe risks to your pet, including but not limited to:

  • Dangerous Wildlife: The pond is a natural habitat for aggressive wildlife, including large snapping turtles, which can cause severe or fatal injuries to dogs.
  • Water Contaminants: Natural water bodies can contain hidden hazards such as harmful algae blooms, bacteria, parasites, and other waterborne contaminants that can cause severe illness or death if ingested or brushed against.

By signing below, you agree to keep your dog completely out of the pond and assume 100% of the medical and safety risks if your dog enters the water.

3. Prohibited Activities (What NOT to Do Inside the Park)

To prevent dog fights, resource guarding, and territorial aggression, the following activities are strictly prohibited inside the fenced dog park:

  • No Grooming or Brushing: Do not brush, clip, or groom your dog inside the park. Loose fur and grooming tools can cause anxiety or territorial behavior in other dogs.
  • No Food, Treats, or Chews: Absolutely no human food, dog food, training treats, bones, or chew toys are allowed inside the gates. Even the best-behaved dogs can become aggressive when food is present.
  • No Personal Dog Toys: Do not bring tennis balls, frisbees, or squeaker toys from home if other dogs are present, as this frequently triggers intense possessive aggression ("resource guarding").
  • No Strollers or Children's Toys: Wheeled strollers, children's toys, and bicycles are forbidden inside the off-leash area.

4. General Dog Park Rules & Regulations

  • Supervision: Dogs must be within unobstructed sight and under voice/signal control of their handler at all times. Dogs may never be left unattended.
  • Leash Requirements: Dogs must be kept on a secure leash (maximum 6 feet) when entering and exiting the park. Collars must remain on inside the gate, but pinch, choke, or spike collars are strictly prohibited during off-leash play.
  • Capacity Limit: A maximum of two (2) dogs per adult handler is permitted in the dog park at one time. Handlers must be at least 18 years of age. Children under 12 are not permitted inside the fence without close adult supervision.
  • Health & Vaccinations: All dogs entering the park must be legally licensed and fully up-to-date on all required vaccinations, including Rabies, DHPP, and Bordetella. Puppies under 4 months of age and female dogs in heat are strictly prohibited.
  • Aggressive Behavior: If a dog displays any signs of aggression, snapping, or excessive barking, the handler must immediately leash the dog and remove them from the park area.
  • Sanitation: Handlers must promptly clean up their dog’s waste and dispose of it in the designated sanitary receptacles.

5. Release of Liability & Indemnification

By entering the dog park area, I explicitly agree to the following: I voluntarily assume all risks associated with my participation and my dog’s participation in the dog park and near the pond area, including the risks of property damage, wildlife attacks, illness from water contaminants, severe physical injury, or death.

I hereby release, waive, and forever discharge Sleepy Hollow Lake Campground, its owners, operators, and employees from any and all claims, demands, or liabilities arising out of or related to my use of this facility. I further agree to indemnify and hold harmless the campground from any claims or financial losses (including legal fees) resulting from damages or injuries caused by my dog to any person, pet, or campground property.


  1. I agree my pet must be current with ALL required shots and immunizations. Please note that only one document can be uploaded per pet. If you have multiple pets, you must fill out corresponding documents to each pet.
  2. I agree my pet will be allowed at the above site/cabin and not those of other guests or at any cabins that do not permit pets
  3. I agree the minimum-security deposit for my pet inside a cabin rental will be left at check in and may not be refundable if there's extra cleaning or damage
  4. I agree that the cost for repair for any damages or additional cleaning charges will be charged to my credit card accordingly, even if discovered after departure. This includes flea treatment etc.
  5. I agree the maximum of pets per site/cabin are 2
  6. I agree to keep my pet out of the pool/swimming/beach/playground/pavilions and other common areas
  7. I agree to keep my pet with me and will not leave the pet in the unit unattended
  8. I agree to walk my pet on the outer area of the property away from the common areas and buildings and will clean up after my pet.
  9. I agree that my pet will not be on any furniture of Sleepy Hollow without protective coverings and the pet owner will be responsible for shedding or damages.
  10. I agree that if my pet is not cleaned up after there will be a fee charged to my account for cleaning
  11. I agree to prevent noise or any other disturbance by my pet, in the interest of other guests. Excessive barking or noise complaints will not be tolerated
  12. I agree that barking, biting and other harmful nuisances will not be tolerated. Should disturbances create complaints, at discretion of management I agree to make arrangements to house my pet elsewhere outside of the campgrounds
  13. Should there be any harm caused by my pet I take full responsibility to the maximum extent and the campground, DBA Sleepy Hollow Lake will not be liable. I understand that if my pet bites someone they will be asked to vacate immediately without refund 
  14. I agree that in the event my pet harms someone, the campground is not responsible, and you agree to pay any and all campground fees should they be incurred from your pet, including lawyer's fees.
  15. I agree and understand should I choose to depart early due to an issue with my pet there will be NO REFUND,


Although Sleepy Hollow does not place a size limit on pets, for the best interest and everyone’s safety NO AGGRESSIVE BREED of dogs will be permitted to stay at Sleepy Hollow or allowed on the property by any visitor or guests. It is the responsibility of their guest to inform the staff of the breed of dog prior to making a reservation. Any dog with a known aggression will not be permitted entry
