CLIENTS ACKNOWLEDGEMENT OF RISKS AND MEDIA RELEASE

By accepting the credit card charges for your trip, you agree to this acknowledgment of risks when booking your trips with Gunnison Valley Anglers Co.

In consideration for my being permitted to participate in the activities coordinated with Gunnison Valley Anglers Co. I agree to the following risks associated with these activities:

 I acknowledge that fishing/boating/hiking has inherent risks, hazards, and dangers for anyone that cannot be eliminated, particularly in a wilderness environment. These inherent risks are some of the same elements that contribute to the unique character of this activity and can be the cause of loss or damage to my equipment, or accidental injury, illness, or in extreme cases, permanent trauma or death. I understand that these risks, hazards, and dangers include without limitation:


1. Water hazards in boating and wading in the rivers including drowning;

2. Hiking in rugged country;

 3. Injuries from fishing equipment and other participants;

4. Encounters with wildlife, animals, and insects;

 5. Temperature extremes;

 6. Inclement weather conditions and unavailability of immediate medical attention in the wilderness in case of injury. I understand the risks, hazards, and dangers of fishing and have had the opportunity to discuss them with my guide and outfitter.

 I understand that these activities may require good physical conditioning and a degree of skill and knowledge. I believe I have that good physical conditioning and the degree of skill and knowledge necessary for me to engage in these activities safely. I understand that I have responsibilities. My participation in this activity is purely voluntary. No one is forcing me to participate and I elect to participate in spite of the risks. I am voluntarily using the services of this outfitter/guide with full knowledge of the inherent risks, hazards, and dangers involved and hereby assume and accept any and all risks of injury, paralysis, or death.

 I certify that I am fully capable of participating in this activity. Therefore, I assume and accept full responsibility for myself, including all minor children in my care, custody, and control, for bodily injury, death, or loss of personal property and expenses as a result of those inherent risks and dangers identified herein and those inherent risks and dangers not specifically identified, and as a result of my negligence in participating in this activity.

 I authorize Gunnison Valley Anglers Co. to take and use any photographs or media in any Gunnison Valley Anglers publication, production or presentation, including electronic/internet marketing material and social media for the purpose of promoting Gunnison Valley Anglers in a positive manner.

 I have carefully read, clearly understood, and accepted the terms and conditions stated herein and acknowledge that this agreement shall be effective and binding upon me, my heirs, assigns, personal representative, and estate and for all members of my family, including minor children.
