Viet San Diego United Waiver and Release of Liability

In consideration of participation in activities organized by Viet San Diego United (the “Club”), the undersigned agrees to the following terms and conditions:

1. Assumption of Risk

I, the undersigned, understand that participating in soccer and related activities involves inherent risks of injury, including but not limited to, sprains, strains, fractures, concussions, or more serious injuries. I acknowledge that I am voluntarily participating in these activities with full knowledge of the risks involved and assume any and all risks of injury, whether known or unknown.

2. Release of Liability

I hereby waive, release, and discharge Viet San Diego United, its organizers, coaches, volunteers, sponsors, and any associated parties from any and all liability, claims, demands, or causes of action that may arise from participation in any activity organized by the Club. This release is valid whether injury or damage is caused by the negligence of the Club or otherwise.

3. Medical Treatment

In the event of an injury, I authorize the Club and its representatives to arrange for any necessary medical treatment. I agree to be responsible for any costs associated with medical care.

4. Code of Conduct

I agree to follow all rules and guidelines set by the Club and understand that failure to do so may result in expulsion from the event or program without a refund.

5. Media Consent

I give permission for Viet San Diego United to take photographs or video of me during events for promotional purposes, including social media, without compensation.

6. Acknowledgment of Understanding

I have read this waiver and fully understand its terms. By signing below, I acknowledge that I am giving up substantial rights, including the right to sue. I sign this waiver freely and voluntarily.
