OUI TOUR MAUI Inc. RELEASE, INDEMNITY, AND HOLD HARMLESS AGREEMENT

If you are 18 years of age or older, you must read, understand, complete and sign this waiver of liability before OUI TOUR MAUI will enroll you for any tours or services.

If you are the parent or legal guardian for children under the age of 18, you must read, understand, complete and sign this waiver of liability on behalf of your child/children before OUI TOUR MAUI will enroll them in any tours or services.

I hereby ,waive all claims for damage or loss to my person or property which may be caused by any act or failure to act by OUI TOUR MAUI, and I assume the risk of all dangerous conditions encountered in connection with any physical activity I may participate in with
OUI TOUR MAUI.
