In consideration for being allowed to participate in the 2025-26 Dancer Auditions event hosted by the Zyia Active Jazz Dancers and related activities held at SMASH Dance Academy (188 West River Park Drive Provo, UT  84604) (the “Event”), the undersigned: (a) acknowledge, fully understand and agree that the Participant will be engaging in activities that involve risks of injury which might result from willful or negligent actions or inactions of the Participant or of others, or the conditions of the premises or of any equipment used, and that there may be other risks not known or not reasonably foreseeable at this time; (b) assume all the aforementioned risk; and (c) release, waive, discharge, covenant not to sue, indemnify and hold harmless Smith Entertainment Group, LLC, Jazz Basketball Investors LLC dba the Utah Jazz, SMASH Dance Academy, all sponsors and advertisers of the Event, and all subsidiaries, affiliates, predecessors, assigns, present and former officers, owners, shareholders, directors, agents and employees of each and every one of the aforesaid entities and persons and the owners and lessees of premises used to conduct the Event (each, a “Released Party”), from any and all liability to the undersigned, their heirs and successors, for any and all claims, demands, losses or damages on account of any injury or loss in any way related to the Event or the participant’s participation therein, including physical injury (and death) and damage to property, caused or alleged to be caused in whole or in party by any Released Party or otherwise. 


The undersigned further grant full permission to the organizers, directors and sponsors of the Event to record the Participant’s likeness and/or voice by any media deemed appropriate, and to use the same in any manner, including publicity, advertising or other commercial purposes, all without payment of any fee or other remuneration of any kind to the undersigned.
