Little Whale Tumble School
ALL Tumble MERS (CHILDREN AND ADULTS) MUST HAVE A WAIVER SIGNED PRIOR TO ENTERING THE SCHOOL. THIS IS REQUIRED!
INFORMED CONSENT AND WAIVER/RELEASE I, the undersigned, as the parent(s) or legal guardian(s) of the child(ren) or as a participant myself listed on the application in consideration of the request and permission of my son(s)/daughter(s) or myself to participate in Little Whale Tumble School’s programs, including, but not limited to Tumble lessons, open play, birthday parties, clinics, and workshops hereby assume full responsibility for all risks of injury or loss which may result from my son(s)/daughter(s) participation in this activity and hereby agree to hold harmless, release and forever discharge Little Whale Tumble School, its officers, agents and employees from and waive any and all claims and demands whatsoever which the undersigned and any of them or any third person of any accident, illness, injury or death of any person and persons, or damage or loss or destruction of any property arising or resulting from Tumbling and/or running indirectly from my son(s)/daughter(s) participation to the aforementioned program and occurring.
Parents of children under 3 must be with their child and over 3 must be supervised by parent.
PHOTOS AND VIDEOS AUTHORIZATION I understand that photos and videos may be taken during my child(ren)’s Tumble class. This media may be used for staff training and/or advertising for Little Whale Tumble School. I may notify a customer service specialist if I wish to exclude my child(ren) from any photos or videos being taken. SOLICITATIONS: Little Whale Tumble Schools prohibit all financial transactions or work arrangements between its employees and customers, including baby-sitting. I agree that while on Little Whale Tumble School property, I will not solicit any Little Whale Tumble School employee for work or services, including baby-sitting, even though the activity will occur off Tumble School property. Employees may not accept any payment, gifts, rewards, monies or gratuities of any kind from a customer. The only exception is for “token” gifts by customers when given solely based upon an employee’s work performance at the Tumble School. (The School believes that financial transactions or other arrangements between its employees and customers, such as babysitting, could create a conflict of interest, give impressions of favoritism, affect employees’ judgements and lead to other negative consequences.)
