Warm Beach Christian Camp & Conference Center Waiver Of Liability And Release PLEASE READ THE FOLLOWING CAREFULLY Disclosure Challenge Course activities often involve warm-ups, games, group initiative problems, low and high challenge elements, a forty foot high climbing tower and other rigorous physical adventure activities. The level of participation in all programs and activities is at all times determined by the participant. While participants’ safety is paramount to Warm Beach Camp, all risk cannot be eliminated from any activity. By choosing to participate, each participant assumes the risk that he/she may incur an emotional or physical injury and/or disability. Injuries can include, but are not limited to; cable burns, rope burns, sprains, skin abrasions, and pulled/strained muscles. In the unlikely event of a failure of a helmet, belay, or other safety precautions, more serious injuries or even death could result. If you have any questions, have them answered before signing this document. Release of Liability I, the undersigned, understand that parts of the Challenge Course experience at Warm Beach Camp may be physically or emotionally demanding. In consideration of being permitted to participate in the Challenge Course at Warm Beach Camp, I, the undersigned, in full recognition and appreciation of the dangers and risks inherent in such activities, do hereby waive, release, and forever discharge Warm Beach Christian Camp and Conference Center, its officers, agents, volunteers, and employees from and against any and all claims, demands, action or causes of action for costs, expenses or damages to personal property or personal injury, or death, which may result from my participation in these activities. I understand and admit that my participation is voluntary. I hereby agree to comply with all rules and regulations. I assume full responsibility for any injuries or damages resulting from my participation in these activities, including responsibility for using reasonable judgment in all phases of participation of the program. I recognize and understand that the activities may be hazardous, that my participation is solely at my own risk, and that I assume full responsibility for any resulting injuries and damages. I affirm that I am in good health. I further declare that I am physically fit and capable to participate in such activities. I understand that it is my responsibility to notify the appropriate person should an injury occur. I acknowledge that I have read and understand this entire Waiver of Liability and Release, and I agree to be legally bound by it.
This Waiver must be presented to the Recreation Staffpriorto participation in the activity. RELEASE: I, parent/guardian, or attendee of 18 years or older, hereby give approval for the attendee listed above to participate in the NWYC at Warm Beach Camp and relieve the Warm Beach Camp, the Seattle Church of Christ, the NW Fellowship of Churches, the rental facility, and all affiliated staff from any and all liability for sickness, accidents or injuries while attending or being transported to/from the facilities. In the event of an emergency and I cannot be contacted, I give consent to the NWYC Director and Medical Director (Nurse) to authorize medical help on site or at an appropriate medical facility. I authorize NWYC staff or nurses to administer prescription and over-the-counter medications sent with the camper and administer First Aid (including over the counter medications, e.g., Tylenol, Benadryl and the like) as required for illness or injury. I give permission to the NWYC Staff to use pictures/video or quotes of my child for the purpose of promoting the camp experience. In accordance with the First Amendment, we reserve the right to our freedom of speech and align our values with the Biblical view of gender. We believe that there are two genders determined by one's physical attributes at birth. As a parent of a camper, you agree to talk with your child about this Biblical standard. We welcome campers that may be struggling with gender dysphoria or other gender-related issues, as we want them to discover the love of God their creator through his Word and his church, however, we reserve the right to organize all campers according to these two genders. By signing this waiver, you waive your right to bring a legal suit for any perceived discriminatory behavior in recognizing these two genders. I do hereby authorize Northwest Youth Camp staff to release information in the event of my child having disciplinary actions at camp. I give permission to release information regarding events taken place at camp to the involved parties, guardian(s), and staff (paid or volunteer) of local congregation(s). Any controversy arising out of, connected to, or relating to any matters herein of the transactions between me and the above named parties or on behalf of the minor child named below, of this Release/Waiver, or the breach thereof, including, but not limited to any claims of violations of Federal and/or State Law, as well as any common law claims shall be settled by arbitration; and in accordance with this paragraph a judgment based upon the arbitrator's award may be entered in any court having jurisdiction thereof. This agreement shall be construed and interpreted under the laws of the State of Washington. The signature of the parent is intended to serve as both liability waiver and medical release. “Nevertheless, conduct yourselves worthy in a manner worthy of the gospel of Christ.” Philippians 2:17 Attendees agree to: - Be considerate, sensitive, and respectful of other attendees, and adults through behavior, language, and dress. This includes obeying direct requests from adult supervisors.
- Refrain from actions that could result in injury and/or property damage.
- Adhere to the stated curfew, whether it’s back to your housing or lights out.
- Attend all scheduled activities, arriving promptly, and staying for the entire event.
- Report any problems (including property damage) to an adult leader.
- Travel in groups of three or more.
- Always tell an adult leader/counselor if you will be leaving the main event area.
Attendees agree to not: - Possess weapons or fireworks of any kind.
- Purchase, possess, consume, or distribute alcohol, illegal drugs, or tobacco and marijuana products and or paraphernalia.
- Engage in any sexual activity of any kind or form of inappropriate romantic activity for example flirting, holding hands, kissing, etc.
- Use profanity or degrading language of any kind.
- Bully or harass others.
- Visit or gather in the sleeping areas of the opposite gender.
- Sneaking out
- Going off-site without adult supervision.
Other: - Adhere to dress code
- You may request a specific cabin/room assignment using the Special Needs box on the first page of the registration form. However, assignments are subject to change as adult leaders see necessary.
- Curfew: 10pm in cabin, 11pm lights out.
- No pranks.
- Need to have an adult present while in public areas after dark.
- Follow phone policy at all times. Phones used outside of this policy will be confiscated.
- Be respectful with camp property.
Parent/Guardian: I agree to instruct my child to abide by all rule and regulations as outlined above. I agree that if my child fails to abide by the Code of Conduct, that my child can be dismissed from the NWYC and sent at my expense for immediate transportation home without refund of any portion of the camp registration fee.
