Liability Waiver & Participation Agreement


By participating in a Puppy Yoga event organized by Puppy Popups LLC, I (the Participant), or I (the Parent or Legal Guardian) on behalf of the Minor(s) listed below, agree to the following:

1. Assumption of Risk and Indemnification

  • I understand that participation in a yoga class involving live animals (puppies) includes inherent risks such as, but not limited to, allergic reactions, scratches, bites, tripping hazards, or other injuries. I voluntarily assume all such risks on behalf of myself and/or the Minor(s) named below.
  • I agree to release, indemnify, and hold harmless Puppy Popups, its owners, agents, employees, instructors, volunteers, affiliates, and facility partners (collectively, the "Released Parties") from any and all liability, claims, demands, or causes of action arising out of or related to participation in the event by myself and/or the Minor(s).

2. Supervision and Behavior

  • I understand that it is my responsibility to supervise any Minor(s) I bring to the event. I agree that all Participants, including children, must follow event guidelines and staff instructions to ensure the safety and enjoyment of all attendees and animals.

3. Photo/Video Release

  • I give permission to Puppy Popups LLC to take and use photos or videos of myself and/or the Minor(s) for promotional and commercial purposes in print, online, or social media, without compensation. I waive any rights to inspect or approve the final use of such media.

4. Eligibility and Acknowledgment

  • I confirm that I am 18 years of age or older and legally authorized to sign this release on behalf of the Minor(s) listed.
  • I have read and understood this agreement, and by checking the box below, I freely accept and agree to its terms for myself and/or any Minor(s) I am registering.

By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.
