Liability Waiver and Release



Review TruBorn Strong, LLC Privacy Policy

I hereby acknowledge that the personal training, coaching and/or gym membership for which I am registering is a strenuous activity which is beyond the capability if some people and may cause minor, severe and/or permanent injuries or death to those people that are not in sufficient physical fitness, training and/or experience.

I Agree

This agreement releases TruBorn Strong, LLC from all liability relating to injuries that may occur from weight training, powerlifting, fitness training, the use of showers, restrooms, and all other activities while attending the TruBorn Strong, LLC facility at 5356 McGinnis Ferry Road, Suite 300, Alpharetta, GA 30005.

I have made my own determination as to whether or not I am able to safely participate in such training and/or coaching. I recognize that Siraj Saeed, TruBorn Strong, LLC have not evaluated my ability to participate in such training and access to facility.

I also recognize that any advice dispensed through Siraj, TruBorn Strong, LLC may not be appropriate for me, and it is my responsibility to make this determination.

I acknowledge that personal training and/or coaching are an extreme test of a person's physical and mental limits and carries with it, regardless of physical fitness or experience, the potential for death, serious injury and property loss. I hereby assume the risks of participation in such training and/or coaching.

I certify that my level of physical fitness is appropriate in the personal training and/or coaching which I am voluntarily undertaking.

I certify that I have not been advised against participation in such by a qualified health care professional.

I understand that TruBorn Strong, LLC may take photographs, audio or recordings, or testimonial accounts that may contain the name, image voice likeness or account of me (collective "images") during the use of the Fitness Services. I hereby irrevocably consent to and grant TruBorn Strong, LLC the exclusive, worldwide, perpetual, royalty-free and otherwise unlimited right to use, copy modify distributes, publicly display and perform, publish, transmit, remove, retain, repurpose, and commercialize any and all such images in any and all media or form of communication whether now existing or hereafter developed, without obtaining additional consent, without restriction or notification, and without compensating me in any way and to authorize others to do the same.

I, for myself, my past, my present and future agents, attorneys, representatives, predecessors, successors, assigns, heirs and executors, do hereby waive, release and discharge Siraj, TruBorn Strong, LLC , its past, present and future owners, directors, offices, employees, agents, representatives, contractors, attorneys, any participants in Siraj Saeed, TruBorn Strong, LLC personal training and/or coaching other than myself, parent, subsidiaries, predecessors, successors, affiliates, assigns, sponsors, any event for which this training and/or coaching is geared or based, and any organizer, sponsor, speaker, volunteer, city, town, village or other municipally or government entity associated with such event (the TruBorn Strong, LLC) from any and all matter of liability, claims for damages, causes of action, suits, proceedings, compensation, attorneys' fees, costs and expenses of suits, claims and demands whatsoever, which may arise in the future from my participation in personal training and/or coaching through Siraj, including, but not limited to any and all claims, losses or liabilities for death, personal injury, partial or permanent disability, property damage, medical or hospital bills, theft or damage of any kind, including economic losses, which may in the future arise out of or relate to my participation in or traveling to and from personal training and/or coaching through TruBorn Strong, LLC, Siraj Saeed

I AGREE NOT TO SUE TruBorn Strong, LLC/Siraj Saeed and for any and all claims made or liabilities assessed against them as discharged herein.

I INDEMNIFY AND HOLD HARMLESS Siraj Saeed/TruBorn Strong, LLC and from any and all claims made or liability assed against them as a result of (i) my actions and inactions (ii) the actions, inactions or negligence of others including those parties hereby indemnified (iii) the conditions of the facilities, equipment or areas where event or activity is being conducted and (iv) any other harm caused by an occurrence related to personal training and/or coaching through TruBorn Strong, LLC
