INDEMNITY WAIVER

By my signature, I acknowledge and agree to the following:

1. That I have been made aware of the safety rules and guidelines for this facility. 

2. That I am solely responsible for my safety and or that of any minor child in my care or custody and voluntarily assume for myself and or minor child, all the risks associated with the use of this facility, including risks caused by another user.

3. I fully understand that roller skating and associated activities involve inherent and foreseeable risks, including but not limited to falls, collisions, physical injury, property damage and in rare instances, serious injury or death.

4. That for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, I indemnify, release, and hold harmless Bowling World Pvt Ltd, or its agents or employees, for any harm and injury that may befall me and or the minor child from the use of this facility.

5. That I consent on my behalf and or that of any minor child in my care or custody, to any medical treatment that may be administered by the staff of Bowling World Pvt Ltd, its assignees or agents, as a result of any accident, injury, or illness arising from the use of this facility and I indemnify, release, and hold harmless Bowling World Pvt Ltd, or its agents, third parties or employees for administering such medical attention, even in circumstances where the parties are negligent. 

6. That, if I or any minor or child in my custody or care is injured or requires medical assistance, Bowling World Pvt Ltd may, at my cost, arrange medical treatment and emergency evacuation as deemed necessary by them, and I agree that I am responsible for my own medical and ambulance insurance cover. 

I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THIS WAIVER AND SIGN THE SAME FREELY AND VOLUNTARILY
