Parent/Guardian Authorization for Photography, Video, Livestreaming, Audio, and Media Use

NOTICE TO PARENT OR LEGAL GUARDIAN

No Backboard Basketball League creates and distributes media as part of its basketball, athlete-development, event, promotional, and storytelling activities.

This may include livestreams, game footage, training footage, photographs, interviews, podcasts, highlight clips, social-media content, promotional materials, statistics, graphics, and other media.

THIS FORM IS SEPARATE FROM THE ATHLETE PARTICIPATION WAIVER (Form 1B).

Signing this Media Release authorizes the uses described below.

This Release does not by itself authorize No Backboard Basketball League or a sponsor to enter the Minor Athlete into a separately compensated personal endorsement, advertising, or NIL agreement. Those opportunities may require a separate written agreement.

1. Parent/Guardian Authorization

I represent that I am the parent or court-appointed legal guardian of the Minor Athlete identified above and have authority to provide this authorization.

I voluntarily authorize No Backboard Basketball League, LLC and the applicable operator of No Backboard Basketball Gym, together with their authorized employees, contractors, production personnel, media partners, platforms, and service providers (“NBBL”), to photograph, film, livestream, record, interview, and otherwise capture the Minor Athlete in connection with authorized NBBL activities.

I understand that the Minor Athlete’s name, image, likeness, voice, athletic performance, statements, and other identifying characteristics may have legal and commercial value.

I knowingly authorize the uses described in this Release, subject to the training-clip and showcase elections below.

2. Media Covered by This Release

“Media” includes photographs, video, livestreams, audio, interviews, podcasts, game footage, training footage, practice footage, behind-the-scenes content, highlight clips, promotional footage, graphics, statistics displays, scoreboards, performance reports, and other audiovisual or digital content created or captured in connection with NBBL activities.

Media may include footage from practices and training sessions; recreational play and open runs; No Backboard Basketball scrimmages; games, leagues, showcases, and tournaments; camps, clinics, tryouts, and competitions; athlete-development sessions; interviews and podcasts; promotional or community events; award presentations; demonstrations; behind-the-scenes activities; and other authorized NBBL programming.

3. Rights Granted

I grant NBBL a non-exclusive, worldwide, royalty-free license to use authorized Media containing the Minor Athlete’s: name; approved display name; image; likeness; voice; athletic performance; interview statements; uniform or apparel as captured; team affiliation when appropriate; basketball statistics and performance information intended for public display; and biographical information voluntarily provided for publication.

NBBL may reproduce, edit, crop, caption, combine, format, publish, display, transmit, stream, distribute, archive, repost, and otherwise use such Media for the purposes authorized by this Release, subject to the elections in Sections 19 and 20.

4. Authorized Uses

Media covered by this Release may be used for legitimate NBBL purposes, including: livestreaming NBBL games and events; game broadcasts and replay; highlight videos and athlete clips; social-media content; websites and mobile applications; YouTube and other streaming platforms; podcasts and interviews; press and public-relations materials; news releases; presentations and pitch materials; fundraising and community-impact materials; NBBL advertising and promotion; promotion of NBBL programs, facilities, leagues, events, and competitions; promotional trailers and sizzle reels; documentary and reality-style programming; athlete-development and instructional content; newsletters and email marketing; printed promotional materials; historical and archival use; and sponsor-supported NBBL programming as described below.

Media may be distributed through current or future forms of print, broadcast, digital, social, streaming, mobile, or online media.

5. Livestreaming and Event Coverage

I specifically understand that NBBL events may be livestreamed or recorded for later distribution.

Livestreams may include the Minor Athlete competing; the Minor Athlete’s name; game statistics; scores; commentary; team identification; replays; highlights; crowd or bench footage; and other normal elements of a sports broadcast.

Because livestreams are transmitted in real time, NBBL cannot retrieve or erase copies that viewers, platforms, media outlets, or other third parties may independently save, repost, screenshot, share, or distribute.

6. Editing and Creative Production

NBBL may edit Media for length, format, clarity, storytelling, branding, graphics, music, commentary, captions, statistics, highlights, or other legitimate production purposes.

Editing may include ordinary automated and artificial-intelligence-assisted production tools for tasks such as clipping; camera tracking; transcription; captioning; statistics; indexing; search; graphics; editing assistance; content organization; and highlight generation.

NBBL will not knowingly edit Media in a manner intended to falsely portray the Minor Athlete as making a statement or engaging in conduct that did not occur.

7. Artificial Intelligence and Synthetic Likeness

This Release does not authorize NBBL to create a synthetic replacement of the Minor Athlete’s identity for unrelated commercial purposes.

Without a separate written authorization, NBBL will not intentionally use the Minor Athlete’s Media to: clone the Minor Athlete’s voice; create a synthetic speaking avatar of the Minor Athlete; generate fabricated statements attributed to the Minor Athlete; create a digital replica intended to substitute for the Minor Athlete in a commercial performance; or license the Minor Athlete’s identity as training material for a general-purpose commercial generative-AI model.

This limitation does not prohibit ordinary AI-assisted production, analytics, clipping, statistics, search, or editing of authentic Media as permitted by Section 7.

8. Sponsor-Supported Content

I understand that NBBL programming may be supported by advertisers, sponsors, partners, or commercial brands.

This Release authorizes the Minor Athlete to appear incidentally or as part of a team, game, event, broadcast, highlight package, or NBBL promotional production that contains sponsor identification, advertising, branded graphics, product placement, or commercial sponsorship.

For example, an NBBL game broadcast may state that the event is “presented by” a sponsor while showing participating athletes.

This Release does not by itself authorize NBBL or a sponsor to represent that the Minor Athlete personally endorses, recommends, or serves as a paid spokesperson for a particular product, service, or company.

A material individual endorsement, paid promotion, individually negotiated NIL campaign, or similar commercial arrangement involving the Minor Athlete should be governed by a separate written agreement when appropriate.

9. NIL Compensation and Separate Opportunities

I understand that appearing in ordinary NBBL event coverage, livestreams, highlights, promotional materials, or other Media authorized by this Release does not automatically entitle the Minor Athlete or Parent/Guardian to payment, royalties, residuals, licensing fees, or other compensation.

However, this Release does not waive the Minor Athlete’s right to negotiate compensation for a separate NIL, endorsement, sponsorship, licensing, creator, merchandise, affiliate, or other commercial opportunity.

Any such compensated arrangement may be documented separately and may be subject to applicable eligibility rules, school rules, association rules, governing-body requirements, or law.

Nothing in this Release promises the Minor Athlete an NIL opportunity or compensation.

10. Ownership of Media

Except for rights in the Minor Athlete’s identity that remain with the Minor Athlete, Media created by or for NBBL may be owned by NBBL or the applicable creator or production partner as provided by law or contract.

The Parent/Guardian and Minor Athlete do not acquire ownership of a photograph, recording, broadcast, graphic, or production merely because the Minor Athlete appears in it.

Nothing in this Release transfers ownership of the Minor Athlete’s general name, image, likeness, voice, or personal identity to NBBL. NBBL receives only the permissions and licenses stated in this Release.

11. Athlete-Created Content

This Release does not automatically transfer ownership to NBBL of photographs, videos, music, artwork, written material, or other original content independently created and owned by the Minor Athlete.

If the Minor Athlete separately submits original content to NBBL for publication, the rights associated with that content may be addressed through platform terms, submission terms, or a separate agreement.

12. Privacy and Personal Information (P4)

This Release does not authorize NBBL to publicly disclose sensitive personal information merely because the Minor Athlete has signed a Media Release.

Absent separate authorization or a legitimate operational reason, NBBL should not intentionally publish information such as the Minor Athlete’s: home address; personal telephone number; private email address; medical information; insurance information; financial information; school schedule; precise private-location information; or other sensitive nonpublic information.

Public athlete profiles may contain ordinary sports-related information such as name, team, position, graduation year, height, statistics, achievements, interviews, and other voluntarily supplied athlete information where appropriate.

Privacy Policy reference: I acknowledge that NBBL maintains a Privacy Policy describing how personal information may be collected and used in connection with NBBL activities. 13. No Approval Right Over Routine Media

I understand that NBBL may produce a significant volume of content and that requiring individual approval before each publication would make normal sports coverage impractical.

Accordingly, I waive any requirement that NBBL submit routine Media covered by this Release to me or the Minor Athlete for inspection or approval before publication.

This provision does not expand NBBL’s rights beyond the uses authorized in this Release.

14. Term and Withdrawal of Future Consent

This Release becomes effective when signed.

The Parent/Guardian may withdraw authorization for future intentional capture and future new promotional uses by delivering written notice to NBBL at info@nobackboardbasketballgym.com(or such other contact method as NBBL designates in writing or on its website).

Withdrawal is prospective only and does not require NBBL to recall Media already distributed; remove third-party reposts; retrieve previously delivered publications; erase historical game broadcasts or event archives; alter completed productions; remove the Minor Athlete from previously created team or group content; or undo uses lawfully made before withdrawal became effective.

After withdrawal becomes effective, NBBL will not intentionally create new individually focused promotional Media featuring the Minor Athlete unless another authorization applies.

If an Activity cannot reasonably be separated from filming or livestreaming, NBBL may limit the Minor Athlete’s participation in that filmed portion rather than guarantee that the athlete will never appear on camera.

Note: Showcase livestream AGREE status (Section 20) is a participation condition for showcases and is not withdrawn by ordinary media-withdrawal notice alone; a change of showcase election must be communicated in writing and takes effect for future showcases only.

15. Release Concerning Authorized Media Use

To the maximum extent permitted by Arizona law, the Parent/Guardian, individually and to the extent legally permitted on behalf of the Minor Athlete, releases NBBL from claims arising from the authorized capture, editing, publication, distribution, display, or use of Media in accordance with this Release, including claims based upon right of publicity, appropriation, or similar rights concerning the authorized use of the Minor Athlete’s identity.

This Release does not authorize knowingly defamatory material, intentionally false endorsements, unlawful use of private information, or uses outside the rights granted by this Agreement.

16. Relationship to Athlete Participation

This Media Release is separate from the Minor Athlete Participation Consent and Liability Agreement (Form 1B).

Signing the liability/participation agreement does not automatically grant media rights. Signing this Media Release does not guarantee participation in any NBBL program or event.

If this Media Release is not signed, the Minor Athlete may participate in activities that NBBL determines can reasonably be conducted without intentional filming of that athlete.

Some livestreamed games, showcases, media productions, or other filmed activities may require a Media Release as a condition of participating in the filmed portion because removing a single participant from the recording may not be operationally practical.

17. Governing Law and Severability

This Release is governed by Arizona law. Venue for disputes relating to this Release shall be in Maricopa County, Arizona, except where applicable law requires otherwise.

If any provision is held invalid or unenforceable, that provision shall be enforced to the maximum extent legally permissible and the remaining provisions shall remain effective.

This Release constitutes the agreement concerning the Media rights described herein unless supplemented or replaced by another written agreement concerning a specific NIL, endorsement, media, sponsorship, or commercial opportunity.

Electronic signatures and counterparts are permitted.

18. Training-Clip Distribution Election (P2 — soft opt-out)

NBBL training sessions are filmed for coaching use and for NBBL to create clips. My child may participate in filmed training whether or not I authorize NBBL to distribute clips featuring my child. If I do not grant the authorization below, NBBL will use reasonable steps to ensure that my child is not featured, identified, or included in any NBBL distributed clip. This does not guarantee that my child will never appear incidentally in wide-angle or background footage.

19. Showcase Livestream / Recording Condition (P1 — hard condition)

NBBL showcases are livestreamed and recorded. My child may participate in a showcase only if I agree that my child may be captured in the livestream and recording, including as an identifiable player. If I do not agree, my child may not participate in a showcase.

I HAVE READ THIS MEDIA RELEASE.

I UNDERSTAND THAT THE MINOR ATHLETE MAY BE PHOTOGRAPHED, FILMED, LIVESTREAMED, RECORDED, INTERVIEWED, AND FEATURED IN NBBL MEDIA.

I UNDERSTAND THE RIGHTS I AM AUTHORIZING AND THE LIMITATIONS OF THAT AUTHORIZATION, INCLUDING THE TRAINING-CLIP AND SHOWCASE ELECTIONS ABOVE.

I UNDERSTAND THAT ORDINARY NBBL MEDIA COVERAGE DOES NOT AUTOMATICALLY CREATE A RIGHT TO NIL COMPENSATION, AND THAT A SEPARATE PERSONAL ENDORSEMENT OR COMPENSATED NIL OPPORTUNITY MAY REQUIRE A SEPARATE AGREEMENT.
