By entering Moisty Ramp LLC, you acknowledge that the facility may record video or take photographs for:

If you do not wish for yourself or your minor child to appear in promotional content, notify us in writing at the contact listed below.

Payments are processed through secure, PCI-compliant third-party payment processors.

We do not store full credit or debit card numbers.

• Emergency medical personnel

WAIVER AND RELEASE OF LIABILITY AGREEMENT

1. Acknowledgment of Risk

I, the participant (or parent/legal guardian if the participant is under 18), acknowledge and understand that skateboarding, scooter riding, and inline skating are inherently dangerous activities. Risks include, but are not limited to, falls, collisions, equipment failure, severe injury, paralysis, and death. I voluntarily accept and assume all risks associated with participation in these activities within Moisty Ramp LLC.

I understand that Moisty Ramp LLC staff may be trained to provide basic first aid for minor injuries, but are not medical professionals. In the event of an injury, I acknowledge that the park staff may provide initial first aid treatment or assist in seeking medical care, but they are not responsible for any further medical treatment or the outcome of any medical care provided.


2. Waiver and Release of Liability

In consideration for being permitted to use the facilities at Moisty Ramp LLC, I hereby:

• Release and discharge Moisty Ramp LLC, its owners, employees, staff, volunteers, contractors, and representatives from any and all liability, claims, demands, lawsuits, or causes of action arising out of any injury, death, or property damage related to participation in activities at the park.


3. Protective Gear Requirement

I understand that helmets, knee pads, and elbow pads are mandatory while using the facilities at Moisty Ramp LLC. I acknowledge that wearing additional protective gear—such as wrist guards, padded shorts, or other recommended equipment—may further reduce the risk of injury.

I accept full responsibility for any and all injuries, accidents, or damages that occur as a result of not wearing mandatory or recommended protective gear, whether due to my own choice, negligence, or failure to comply with park rules. I agree that Moisty Ramp LLC, its owners, employees, and agents are not liable for injuries or harm sustained because of my failure to properly wear protective equipment.


4. Rules and Conduct

I agree to follow all posted rules, guidelines, and safety instructions provided by Moisty Ramp LLC and its staff. I understand that these rules are in place to promote a safe environment for all participants.

I acknowledge that failure to follow park rules or staff instructions may result in my immediate removal from the premises without refund, and that additional disciplinary actions or restrictions may be imposed as deemed necessary by park staff.


5. Media Release

I give Moisty Ramp LLC permission to photograph or record me (or my minor child) during activities at the park. I understand that these images may be used for promotional materials, the website, social media, marketing, or educational content. I authorize Moisty Ramp LLC to use these photos or recordings in any format, now or in the future, without any compensation.


6. Signature and Acceptance

By signing below, I certify that I have read and fully understand this agreement, and I sign it voluntarily. I acknowledge this document is legally binding.
