1. This agreement is between the customer named below and the boom truck owner, Superior Charters, Inc. hereinafter referred to as “Superior Charters.” The customer and Superior Charters enter into this agreement subject to terms and conditions herein. 2. All Superior Charters boom truck service invoices include a start-up fee ($250) plus hourly rate ($210/hr). Billing is “port-to-port,” meaning billing time begins when the truck operator leaves the Superior Charters garage to go to the scheduled job site and ends when the boom truck operator returns the boom truck to the garage. 3. A deposit of 2 billable hours is required for jobs estimated at greater than 6 hours. 4. In the event that a boom truck service is scheduled and agreed upon between Superior Charters and the customer named below is not present when the truck operator arrives at the jobsite, or there are complications at the job site prohibiting the truck operator from completing the agreed upon work, Superior Charters reserves the right to charge a "show-up fee" regardless of whether or not the work is completed. 5. Boom Truck max weight is 8,500lbs, max reach is 85'. In the event that a boom truck service is scheduled and agreed upon and when the truck operator arrives at the jobsite it is found that the job cannot be completed due to max weight, max reach, or other foreseeable circumstances not accounted for by the customer, Superior Charters reserves the right to charge a "show up fee" regardless of whether or not the work is completed. 6. The “show-up fee” charge is equal to one-hour of boom truck service ($210), plus start-up fee ($250), plus the truck operator’s time spent driving to and from a particular job site. 7. Invoices are due 14 days from invoice date and must be paid in full by due date, payment is accepted via cash, check, or credit card (processing fee applies). Late fee charges will be added at a rate of 5% each week payment is late. 8. Superior Charters services include boom truck and driver, additional crew required for job must be supplied by the customer. 9. Customer is responsible for safety on the job site. The undersigned herby agrees to indemnify and hold harmless Superior Charters, Inc., its insurers, any contracted boom truck operators, affiliates and employees harmless for any death, injury or damage to property resulting from boom truck use. Superior Charters cannot be held responsible for any lost job time or wages resulting from truck maintenance issues leading up to or during contracted job. Customer states that he/she has read and understands the provisions of this agreement and by signing, agrees to the terms of this agreement in full. |
