**Participant Liability Waiver for Stained Glass Activities**

THIS LIABILITY WAIVER AND RELEASE (hereafter referred to as the “Agreement”) is a legal document that outlines the risks associated with participation in stained glass activities and releases the Activity Provider from liability.

ASSUMPTION OF RISKS

By signing this Agreement, the Participant acknowledges that they are aware of and understand the potential risks associated with stained glass activities, including but not limited to:

  • Exposure to hazardous materials such as lead and chemicals used in the glassworking process.
  • Risk of burns from soldering irons and other hot equipment.
  • Physical risks associated with handling sharp glass and heavy tools.
  • Potential injury from falling objects and use of equipment.

MEDICAL CONSIDERATIONS

This activity is not suitable for individuals who are pregnant, nursing, or have any medical conditions that could be exacerbated by exposure to materials or risks associated with stained glass activities. By participating, the Participant confirms they do not fall into these categories and possess the necessary physical ability to engage in the class safely.

Please be aware that shaky or arthritic hands may make it difficult to fully participate in certain activities. If you are unsure about your ability to join safely, we encourage you to reach out before registering so we can discuss whether the workshop will be a good fit for you.

CANCELLATION POLICY

Payment is due in full at the time of booking to reserve your space.

Cancellations made at least 7 days prior to the workshop will receive a full refund.

Cancellations made less than 7 days prior to the workshop will receive a 70% refund, as 30% of the fee is non-refundable to cover materials and preparation costs.

Thank you for understanding and supporting the time and resources that go into preparing each workshop.

AGE RESTRICTIONS

Participants must be at least 13 years of age. Children under this age are not permitted to participate in stained glass activities due to the nature of the materials and tools used.

LIABILITY RELEASE

In consideration of being allowed to participate in stained glass activities, I, on behalf of myself and my heirs, release and hold harmless the Activity Provider, its employees, agents, and representatives from any and all liabilities, claims, or causes of action that may arise from my participation in such activities, including but not limited to personal injury or property damage, even if caused by the negligence of the Activity Provider.

Acknowledgment

By signing this Agreement, I acknowledge that I have read and understood this waiver and release, and I voluntarily agree to its terms. I understand that my agreement to this waiver is a prerequisite for being allowed to participate in stained glass activities.
