Hands On Deck Virtual Program Participation Agreement


Acknowledgment of Participation: By signing up for this program I understand that it is a four day commitment for all students registered. By participating in Hands On Deck (HOD) summer camp programs and activities, I understand that the program involves the use of basic tools and materials. I acknowledge that there may be some risks involved, such as minor cuts, scrapes, or other injuries that can occur when handling these materials.

Parental/Guardian Certification: I certify that I am the parent or legal guardian of the student enrolled in this program and that I give permission for their participation. 

Assumption of Risk: I understand that while HOD takes care to ensure projects are safe and appropriate, some risks cannot be eliminated entirely. By participating, I voluntarily accept these risks and take responsibility for ensuring that proper safety measures are followed.

Conduct and Responsibility: I understand that students are expected to follow all provided instructions and guidelines during lessons. 

Liability Waiver: In consideration of the opportunity for my student to participate, I agree to release Hands On Deck, its staff, volunteers, and directors from any claims for injury, damage, or loss that may result from participation, except in cases of gross negligence or intentional misconduct.

Media Release: I consent to the use of photos, video, or audio recordings of my student’s participation for promotional purposes. If I do not wish to grant this permission, I will notify HOD in writing.

Additional Content: During summer camps parents/guardians are responsible for providing lunch for their student. If Hands on Deck (HOD) must provide food to a student there will be a $15 dollar charge for each meal.

If student is not picked up from camp a $15 dollar charged will be applied for each hour after the agreed upon pick up time.
