ASSUMPTION OF RISK, RELEASE & INDEMNITY AGREEMENT

Waiver, Release of Liability, and Hold Harmless Agreement

JD Echelon, LLC Firearms Training

State of Nevada

This Agreement is entered into as of the date of signature by the undersigned participant (hereinafter “Participant”) in favor of JD Echelon, LLC (hereinafter “Company”), its officers, employees, agents, contractors, and affiliates.

WHEREAS, Participant has voluntarily elected to participate in firearms training, instruction, and/or related activities offered by the Company (hereinafter “Activities”);

AND WHEREAS, Participant understands and acknowledges that firearms training involves inherent risks, including but not limited to, the risk of serious injury, death, and property damage;

Participant hereby agrees as follows:

1. Acknowledgment and Assumption of Risk

Participant acknowledges that:

1.1 Participation in Activities involves known and unanticipated risks that could result in physical or emotional injury, death, or damage to themselves, third parties, or property.

1.2 Such risks include, but are not limited to, accidental discharge of firearms, equipment failure, negligence by other participants, exposure to environmental conditions, and other risks inherent in the handling of firearms.

1.3 Participant voluntarily assumes all such risks and accepts full responsibility for any injuries or damages that may occur as a result of their participation in the Activities.

2. Waiver and Release of Liability

Participant, on behalf of themselves, their heirs, executors, administrators, and assigns, hereby releases, waives, and discharges the Company, its officers, employees, agents, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action for personal injury, property damage, or wrongful death arising out of or connected with their participation in the Activities, whether caused by negligence or otherwise.

3. Indemnification and Hold Harmless

Participant agrees to indemnify, defend, and hold harmless the Company, its officers, employees, agents, contractors, and affiliates from any and all claims, liabilities, damages, or costs, including attorney fees, arising out of:

3.1 Participant’s participation in the Activities;

3.2 Any act or omission by Participant, including but not limited to negligence, while participating in the Activities.

4. Medical Treatment

Participant consents to receive medical treatment in the event of injury during Activities if deemed necessary by the Company or medical professionals. Participant assumes full responsibility for any medical expenses incurred as a result of such treatment.

5. Firearms Handling and Safety

Participant acknowledges and agrees to comply with all safety rules, instructions, and range commands provided by the Company or its instructors. Failure to adhere to safety protocols may result in termination of participation without refund.

6. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Nevada. Any disputes arising under or in connection with this Agreement shall be subject to the exclusive jurisdiction of the courts of Nevada.

7. Severability

If any provision of this Agreement is found to be unenforceable or invalid, the remainder of the Agreement shall remain in full force and effect.

8. Entire Agreement

This Agreement constitutes the entire understanding between the Participant and the Company regarding the subject matter herein and supersedes any prior agreements or understandings.

Participant Acknowledgment

By signing below, Participant certifies that they:

• Have read and understand this Agreement.

• Are 18 years of age or older and fully competent to sign this Agreement.

• Understand that they are giving up substantial rights, including the right to sue.

• Agree to participate voluntarily and assume all risks associated with the Activities.
