1. DEFINITIONS Use same definitions as Facility Waiver, with the following addition: “FIREARMS ACTIVITIES” means live-fire shooting, firearms handling, firearms instruction, range participation, ammunition handling, holster work, force-on-force training, firearm rentals, competitions, and related activities.
2. FIREARMS ELIGIBILITY ACKNOWLEDGMENT Parent/Guardian acknowledges: - Minor Participants under age 10 are prohibited from participating in live-fire firearms activities unless expressly authorized in writing by RELEASED PARTIES
- Minor Participant must comply with all age restrictions, supervision requirements, and instructor directives
- RELEASED PARTIES reserve the right to deny participation at any time
I authorize Minor Participant age 10 or older to participate in live-fire firearms activities under required supervision. I Agree
3. PARENTAL CONSENT I certify that: - I am the parent or legal guardian of the Minor Participant
- I have legal authority to execute this Agreement
- I consent to the Minor Participant participating in FIREARMS ACTIVITIES
- I understand the heightened risks associated with firearms activities
4. ACKNOWLEDGMENT AND ASSUMPTION OF RISK Parent/Guardian understands that FIREARMS ACTIVITIES involve inherent and significant risks including: - Gunshot wounds
- Ricochets
- Firearm malfunction
- Ammunition malfunction
- Hearing damage
- Lead exposure
- Burns
- Physical injury
- Death
- Negligent or accidental discharge
- Instructor or participant error
- Third-party conduct
- Exposure to loud noise and hazardous materials
Parent/Guardian voluntarily assumes all risks associated with Minor Participant’s participation in FIREARMS ACTIVITIES.
5. FIREARMS SAFETY ACKNOWLEDGMENT Parent/Guardian acknowledges: - FIREARMS ACTIVITIES require strict adherence to safety procedures
- Minor Participant must immediately comply with all instructor and Range Safety Officer commands
- Failure to follow safety rules may result in immediate removal from the FACILITY
Parent/Guardian certifies that Minor Participant is capable of safely following instructions and participating responsibly.
6. RELEASE OF LIABILITY To the fullest extent permitted by law, Parent/Guardian releases and waives claims belonging to Parent/Guardian arising from Minor Participant’s participation in FIREARMS ACTIVITIES. Parent/Guardian acknowledges and agrees that RELEASED PARTIES shall not be liable for injuries arising from ordinary negligence to the fullest extent permitted by law.
7. PARENTAL INDEMNIFICATION Parent/Guardian agrees to defend, indemnify, and hold harmless RELEASED PARTIES from any claims, liabilities, damages, losses, attorney fees, or causes of action brought by or on behalf of Minor Participant arising from: - FIREARMS ACTIVITIES
- Range participation
- Firearm handling
- Violation of safety procedures
- Conduct of Minor Participant
8. SUPERVISION REQUIREMENTS Minor Participant must remain under direct supervision as required by RELEASED PARTIES. Parent/Guardian understands that RELEASED PARTIES are not assuming parental or custodial responsibilities.
9. MEDICAL AUTHORIZATION Parent/Guardian authorizes emergency medical treatment for Minor Participant if deemed necessary and accepts full financial responsibility for resulting medical expenses.
10. PHOTO, VIDEO, AND MEDIA RELEASE Parent/Guardian authorizes RELEASED PARTIES to use Minor Participant’s likeness, image, or recordings for lawful promotional or educational purposes.

By signing this Agreement, Parent/Guardian acknowledges: - The required firearms safety video was reviewed
- Minor Participant understands range safety expectations
- Parent/Guardian agrees to ensure compliance with all firearms safety rules and instructor commands

12. ELECTRONIC SIGNATURE CONSENT Parent/Guardian consents to electronic signatures and electronic records.
13. GOVERNING LAW AND VENUE This Agreement shall be governed by Nebraska law. Any legal action arising from this Agreement shall be brought exclusively in Sarpy County, Nebraska.
14. SEVERABILITY If any provision is found unenforceable, the remaining provisions shall remain in effect.
15. ACKNOWLEDGMENT OF UNDERSTANDING Parent/Guardian acknowledges: - They carefully read this Agreement
- They understand its contents
- They understand the heightened risks of FIREARMS ACTIVITIES
- They had opportunity to consult legal counsel
- They sign voluntarily
