In consideration of being allowed to enter the play space and/or participate in any activity, drop-in play, workshop, party or program and/or use of the property, facilities and services therein, I agree to the following: 

  1. I agree that the participant(s) named below and guardian on premises will comply with all posted safety signs, rules and verbal instructions for admission to Create and Play Kids play space, workshops and activities.
  2. I, for myself and any minor children for whom I am responsible, understand and agree that it is my responsibility to supervise and monitor the safety of the minors in my care while on Create and Play Kids premises.
  3. EXPRESS ASSUMPTION OF THE RISK AND RESPONSIBILITY: I, for myself and any minor children for whom I am responsible and on behalf of my heirs, assigns, personal representative and next of kin, hereby knowingly and freely assume all such risks, both known and unknown, including full responsibility for and risk of bodily injury, or death, or damage to other third parties and their property as a result of my participation in the Activity and/or my use of materials, even if the injury, death or property damage is caused, in whole or in part, by the negligence of Create and Play Kids (CPK) its members, officers, officials, agents and/or employees.
  4. I understand and agree that socks are required to be worn in the play areas.
  5. I understand and agree that food and drink is not allowed in the play areas.
  6. I agree that any violation of this agreement and/or the rules could result in expulsion from the CPK facility with no monetary or other reimbursement.
  7. RELEASE AND WAIVER OF LIABILITIES: I further agree to hold CPK harmless from any and all liability, actions, causes of actions, claims and demands of any kind and nature whatsoever which I now have or may arise from or in connection with the play space, materials and programs.. I further understand that CPK carries no medical insurance for the protection of participants in the play space, and any insurance coverage existing with respect to CPK shall not alter the terms of this waiver nor impose any liability on CPK.
  8. NAME AND LIKENESS: I hereby grant CPK the right to use, for promotional purposes only, any photographs and/or videos taken of me in connection with my participation in the play space and activities..

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY.
