I release the Hopkinton Public Library Foundation, Inc., (HPLF) and its board of directors and volunteers; the Town of Hopkinton and Hopkinton Public Schools and their officers, employees and agents; and all companies, government entities and organizations supplying trucks or vehicles and any of their officers, employees and agents (“Released Parties”) from any and all liability or claims for damages, including personal injury, illness, death, and property damage, that may arise from, or be in any way connected to, my and my child(ren)’s participation in the HPLF Touch-A-Truck and Food Truck Festival event on Sunday, June 1, 2025, at the Hopkinton High School. I understand that this release applies to both present and future injuries, and that it binds myself, participating child(ren), and the heirs, devisees, personal representatives, and administrators of each of these persons. I certify that, as the parent or guardian for my child(ren), I have authority to sign this release and waiver on behalf of my child(ren). I further certify that I have read this waiver in full, understand it, and have signed it voluntarily.

ASSUMPTION OF THE RISK. I acknowledge and understand the following:

  1. Participation includes possible exposure to and illness from infectious diseases including but not limited to COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist;
  2. I knowingly and freely assume all such risks related to illness and infectious diseases, such as COVID-19, even if arising from the negligence or fault of the Released Parties; and
  3. I hereby knowingly assume the risk of injury, harm and loss associated with attending Touch-A-Truck, including any injury, harm and loss caused by the negligence, fault or conduct of any kind on the part of the Released Parties.

BY ATTENDING AND/OR PARTICIPATING IN TOUCH-A-TRUCK, YOU ARE DEEMED TO HAVE GIVEN A FULL RELEASE OF LIABILITY TO HPLF AND THE RELEASED PARTIES.
