NAZA SURF CAMP WAIVER, LIABILITY AND INDEMNITY AGREEMENT

ACKNOWLEDGMENT OF RISKS

In consideration of my child’s participation in the beach activity mentioned below,

I AGREE AS FOLLOWS:

I understand that surfing and related ocean activities carry an inherent risk of bodily injury, including, in extreme cases, loss of life from the surf/swimming environment.

I acknowledge and assume any and all risk associated with the presence of any and all sea life that may be in the ocean or on the beach. 

I hereby release Naza Family Inc. from any and all liabilities, claims, actions, damages,costs, and/or expenses, arising from or in any way connected with my child's participation in ALL activities conducted by Naza Surf Camp.

I agree to indemnify, defend, hold harmless and release from all liability Naza Surf Camp instructors, owners, sponsors, volunteers and the state of Florida from ALL LIABILITY to my child or myself, my family, heirs, assigns, personal representatives or next of kin for ANY LOSS OR DAMAGE RESULTING FROM PHYSICAL OR MENTAL INJURY, DEATH OR PROPERTY DAMAGE arising from my/my child’s participation in all Naza activites.

Naza is not responsible for any lost/stolen or damaged property. Campers assume full responsibility for care of all personal possessions.

I FURTHER AGREE THAT:

• My child is in good health and is an able swimmer.

• Photos taken may be used for promotional purposes.

I HAVE READ AND UNDERSTAND this “Acknowledgment of Risks and Release, Waiver of Liability and Indemnity Agreement”

and have signed this Acknowledgment of risks and Release, Waiver of Liability and Indemnity voluntarily, and agree that no oral representation, agreements, or inducements, apart from the foregoing written agreement have been made.
