New Waiver and Release of Liability. 


I desire to use the facility of Legends Sports Academy, LLC. In consideration for being allowed to workout in or with, or active in or with, and use the facilities of Legends Sports Academy, LLC, I acknowledge and agree that: I understand that participation in the activities risk of serios injury, including, but not limited to paralysis, dismemberment, permanent disability, and death, and other losses, both to my person and property. I understand that these injuries and losses can result not only from my actions, inaction, or negligence, but the actions, in action or negligence of Legends Sports Academy, LLC and its owners, and other individuals working out or participating at the training facility. I also understand that these injuries and losses can result from the condition of the facilities and the equipment used. Additionally, I understand that these injuries and losses can result from not being in proper physical condition to undertake the activities. I assume all risk in any way connected with my participation in the facility and accept all personal responsibility for any injury or loss in any way connected with my participation in the activities at Legends Sports Academy, LLC. I also assume my own responsibility to be in the proper physical condition to undertake the activities.


Assumption of the Risk and Waiver of the Liability Relating to Coronavirus/COVID-19, and /or other Illnesses.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Heath Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Legends Sports Academy, LLC (LSA)has put in place preventative measures to reduce the spread of COVID-19; however, LSA cannot guarantee your risk and your child(ren)’s risk contracting COVID-19 and /or other illnesses.


By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Legends Sports Academy, LLC and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at LSA many result from the actions, omissions, or negligence of myself and others, including, but not limited to, LSA employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the forgoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability of expense, of any kind that I or my child(ren) may experience or incur in connection with I or my Child(ren)’s attendance at the LSA or participation in LSA programming (“claims”). On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless the Legends Sports Academy, LLC, its employees, agents, and I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of LSA, its employees, agents and representatives, whether a COVID-19 infections occurs before, during or after participation in Legends Sports Academy, LLC.


Social Media Release


This form confirms the agreement between you and the Legends Sports Academy, LLC regarding your participation in Legends Sports Academy, LLC in which you may be photographed or videotaped (the Property) from time to time. I, hereby irrevocably grant Legends Sports Academy LLC perpetually, exclusively, and for all media thought-out the world (including print, non-theatrical, home video, DVD, social medical, internet and other electronic medium presently in existence or incented in the future), the right to use and incorporate (alone or together with other materials), in whole or in part, photographs or video footage taken of me as a result of my participation in approved activities for Legends Sports Academy LLC.


I hereby agree that I will not bring or consent to others brining claim or action against Legends Sports Academy LLS on the grounds that anything contained in the Property, or in the advertising and publicity used in connection herewith, is defamatory, reflects adversely on me, violates any other right whatsoever, including, without limitation, rights of privacy and publicity. I hereby release Legends Sports Academy LLC, its directors, officers, successors and assign from and against any and all claims, demands, actions, causes or action, suits, costs, expenses, liabilities, and damages whatsoever that I may hereafter have against Legends Sports Academy LLC in connection with the property.


I WAIVE, RELEASE, HOLD HARMLESS, and PROMISE NOT to file suit against Legends Sports Academy LLC, its owners, directors, trainers, or volunteers for any injury, paralysis, dismemberment, disability, death, or loss of damage in any way connected with my participation in the activities associated with Legends Sports Academy, LLC, or other third party groups, whether or not caused in whole or part by the actions, inactions, or negligence of Legends Sports Academy, LLC, the owners of the facility, or a third party group.


This voluntary waiver and release from liability agreement is to be interpreted consist with the laws of the state. I have read the voluntary waiver and release from liability agreement. I understand that I have given up substantial rights by signing it, and I am signing the waiver and release from liability agreement voluntarily for myself and my child (ren). and paste the body of your waiver here.
