THE GRACE NETWORK VOLUNTEER

ACKNOWLEGMENT OF RISK AND GENERAL RELEASE OF LIABILITY

Thank you for your interest in volunteering with The Grace Network. Our events include the following activities:

-      Carrying and moving packages up to 30 pounds

-      Using box cutters and blades

-      Packing and unpacking toiletries and hygiene items, including goods with scents (soaps, shampoos, personal care items)

Waiver of Liability

The Grace Network does not provide insurance coverage to volunteers. The purpose of this waiver is to provide notice to that volunteers will not have insurance coverage by the The Grace Network and to document the volunteer’s acknowledgment that they are providing volunteer service at their own risk.

You acknowledge that The Grace Network does not provide insurance coverage for any loss, injuries, illness, or death resulting from your unpaid service to The Grace Network.

You agree to assume all risk for death or any loss, injury, illness, or damage of any nature or kind, arising out of your supervised or unsupervised service to The Grace Network. You also agree to waive any and all claims against The Grace Network, its members, employees, agents or assigns, for loss due to death, injury, illness or damage of any kind arising out of your supervised or unsupervised service to The Grace Network.

 You also release, waive, agree not to sue, discharge, and forever hold harmless The Grace Network, its Board of Directors, officers, volunteers, agents, and representatives, of and from any and all claims, liability, causes of action (of any kind or nature), damages, costs and/or expenses arising out of, relating to or in any way connected with your volunteer activities.
