USSSA-sanctioned under U.S. Soccer AMATEUR ATHLETIC WAIVER AND MEDICAL RELEASE This waiver must be completed and accepted by the participant or, for a minor, the participant’s parent or legal guardian. Do not provide personal contact information for anyone under 13. 1. Assumption of Risk. The participant is physically able to play soccer and understands that soccer, event travel, facilities, equipment, weather, and field conditions involve inherent risks of injury, disability, death, property loss, and financial loss. The participant and parent or legal guardian knowingly accepts all such risks, including risks caused by their own conduct or the conduct or negligence of others. 2. Medical Care, Insurance, and Reporting. Trainers, massage therapists, medical personnel, and automated external defibrillators may not be available. In an emergency, call 911. The participant or guardian authorizes qualified medical personnel to provide reasonably necessary emergency treatment and accepts all related costs. The tournament and independent medical providers are not responsible for each other’s acts or services. Each participant must pay the separate $16 insurance and administrative fee. The included USSSA player accident coverage has a $100,000 limit and $500 deductible and is secondary to other collectible insurance, or primary if none applies. It does not cover coaches or spectators. Report injuries, treatment, dangerous conditions, accidents, or incidents promptly by phone or text at 801-616-6009 or by email at info@uscompsoccer.com. 3. Safety and Rules. Before each game, the participant or guardian will inspect the field, facilities, goals, and equipment; report any unsafe condition to a coach and tournament official; and refuse to participate until it is addressed. The participant and guardian agree to follow US Comp Soccer, LLC rules, the code of conduct, and applicable city and school-district requirements available at www.usucs.com. 4. Release and Indemnification. To the fullest extent permitted by law, the participant and guardian release, indemnify, and hold harmless US Comp Soccer, LLC, its affiliates, tournament hosts, sponsors, officials, coaches, referees, schedulers, property owners, cities, schools, school districts, and representatives from claims arising from attendance at or participation in US Comp Soccer, LLC events. 5. Weather, Public-Health Conditions, Cancellation, and Refunds. Events are held from November through March and may involve wet, snowy, cold, dry, dormant, damaged, or otherwise unpredictable fields. Participants and spectators will follow applicable public-health and law-enforcement requirements. Games or events may be modified, shortened, decided by penalty kicks, relocated, terminated, or canceled when weather, field restrictions, pandemics, civil unrest, war, acts of nature, or other circumstances beyond the tournament’s control require it. 6. Media and Communications. Participants and spectators’ consent to the use of their image, voice, or likeness in tournament-related coverage and promotion across television, radio, websites, social media, film, and other media. They also agree to receive communications about this event and related soccer activities and or sponsors and may unsubscribe at any time. 7. Binding Effect and Severability. This waiver applies to the specific event, is intended to be as broad as permitted by law and remains enforceable if any provision is severed. Financial responsibility will first fall on the participant or guardian, then other applicable insurance or team or club resources, and finally the USSSA policy; the USSSA policy is primary only when no other insurance applies. Registration and individual entry fees are nonrefundable. US Comp Soccer, LLC is not liable for resulting inconvenience or economic loss. At its sole discretion, the tournament may offer a credit toward a future event, but no credit is guaranteed. The contingency plan and related rules are available at www.usucs.com. 8. Electronic Acceptance. By electronically signing, the participant or authorized parent or legal guardian confirms that they have read and understand this waiver, voluntarily accept its terms, surrender substantial rights including the right to sue, and agree that it is legally binding and constitutes a complete release of liability to the fullest extent permitted by law. A copy may be printed when signing, obtained at www.usucs.com, or requested at info@uscompsoccer.com. |
