Appearance Release Form - One Voice the Tennessee Mass Choir Documentary


1. Grant of Rights

Participant grants to Producer, its successors, assigns, licensees, and designees the irrevocable right to record, photograph, film, tape, and otherwise capture Participant’s name, image, likeness, voice, performance, and biographical information (collectively, “Likeness”) in connection with the documentary currently entitled One Voice the Tennessee Mass Choir Documentary (the “Project”). Participant grants Producer the exclusive, perpetual (without expiration), worldwide right to use, reproduce, distribute, broadcast, exhibit, advertise, promote, sell, license, and otherwise exploit the Project and Participant’s Likeness, in whole or in part, in all media and formats now known or later developed, including but not limited to theatrical release, broadcast television, streaming services, social media platforms, and any future technologies or forms of video content distribution, without restriction as to manner, frequency, or duration.


2. No Compensation

Participant acknowledges that no compensation, fee, residual, or royalties will be paid for this appearance. Participant waives any claim to further consideration, except as may be separately agreed in writing.


3. No Approval Rights

Participant acknowledges that Producer shall have sole discretion regarding the use of Participant’s Likeness, including without limitation editing, alteration, and contextual integration into the Project. Participant waives any right to inspect or approve the finished Project or its use


4. Release of Liability

Participant releases and holds harmless Producer, its members, employees, contractors, licensees, successors, and assigns from any and all claims, demands, or causes of action arising out of or relating to the use of Participant’s Likeness in the Project, including claims for defamation, invasion of privacy, or infringement of rights of publicity.


5. Representations

Participant represents that: Participant is at least 18 years of age and has the full legal capacity to sign this Release. If Participant is under 18 years of age, the signature of a parent or legal guardian is required below. Participant understands the contents of this Release and signs it voluntarily.


6. Governing Law

This Release shall be governed by and construed in accordance with the laws of the State of Tennessee, without regard to conflicts of law principles.
