Adventure & Activity Participation Waiver

Please read this document carefully before participating in any activities provided by Hazlewood Adventures. By signing this form, you confirm that you understand and accept the terms below.


1. Acknowledgement of Risk

I understand that participation in outdoor and adventure activities involves inherent risks, including but not limited to:

  • Falls from height or uneven terrain
  • Collisions with objects, equipment, or other participants
  • Loss of control of vehicles (including quad bikes)
  • Injuries from tools or equipment (including axes, air rifles, and crossbows)
  • Environmental hazards including weather and natural obstacles

I acknowledge that these risks may result in serious injury or death.


2. Voluntary Participation

I confirm that I am voluntarily participating in these activities and accept all inherent risks associated with them.


3. Duty of Care & Negligence

Hazlewood Adventures will take all reasonable steps to ensure participant safety. Nothing in this agreement excludes or limits liability for death or personal injury caused by negligence.

However, I accept responsibility for risks that are inherent to the activities and not caused by negligence.


4. Compliance with Instructions

I agree to follow all safety instructions provided by instructors at all times. I understand that failure to do so may result in my removal from the activity without refund.


5. Safety Briefings

I understand that I will receive a safety briefing before participating. If I do not receive a safety briefing, I will not continue with the activity and will report this immediately to a member of staff.


6. Health & Fitness Declaration

I confirm that I am physically and mentally fit to participate. I will inform staff before participation if I:

  • Am pregnant
  • Have high blood pressure, heart conditions, or epilepsy
  • Have current or previous back or neck injuries
  • Am under the influence of alcohol or drugs

I understand that failure to disclose relevant medical conditions may increase the risk of injury.


7. Emergency Medical Treatment

In the event of an accident or illness, I consent to receiving first aid and any necessary emergency medical treatment.


8. Age & Activity Restrictions

I agree to comply with all stated age and weight restrictions for activities, including:

  • Quad biking 150cc: minimum age 16, maximum weight 20 stone
  • Quad biking 400cc: minimum age 16, maximum weight 25 stone
  • Air rifles, axe throwing, crossbows: minimum age 12
  • All other activities: minimum age 7 unless otherwise stated


9. Behaviour Policy

I understand that a warning system is in place. Unsafe behaviour may result in warnings and removal from the activity without refund.


10. Personal Property

Hazlewood Adventures is not responsible for loss or damage to personal belongings or clothing.


11. Photography & Media Consent

I understand that photos or videos may be taken during activities and used for promotional purposes. I will inform staff if I do not consent.


12. Parental / Guardian Consent (Under 18s)

For participants under 18, a parent or legal guardian must:

  • Provide consent for participation
  • Confirm the participant is fit to take part
  • Accept responsibility for ensuring the participant follows all instructions


13. Accuracy of Information

I confirm that all information I have provided is accurate and that I will notify Hazlewood Adventures of any changes.


14. Data Protection

All personal data will be stored securely and processed in accordance with UK GDPR and applicable regulations.


15. Agreement

By signing below, I confirm that:

  • I have read and understood this waiver
  • I accept the risks involved
  • I agree to comply with all safety instructions
