The Logo Basketball Shooting Facility – Sarasota, FL 1. Acknowledgment of Risks I acknowledge that participation in basketball training, shooting activities, contests, and related events at The Logo Basketball Shooting Facility (“The Logo”) involves inherent risks, including but not limited to: slips, trips, falls, collisions, overexertion, use of equipment (including Dr. Dish shooting machines), and interaction with other participants. I understand these risks may result in injury, illness, or property damage. 2. Assumption of Risk I voluntarily assume all risks associated with my participation at The Logo, whether known or unknown, including those arising from the ordinary negligence of The Logo, its owners, officers, employees, contractors, or agents. 3. Release of Liability In consideration of being allowed to participate, I hereby release, discharge, and hold harmless The Logo, Hooper Performance LLC, its owners, managers, employees, agents, contractors, successors, and assigns from any and all liability, claims, demands, or causes of action arising out of my participation, to the fullest extent permitted by law. 4. Equipment Use Agreement I agree to use all facility equipment, including Dr. Dish shooting machines, in a safe and responsible manner and only as instructed. I accept full responsibility for any injury resulting from misuse. 5. Medical Authorization In the event of a medical emergency, I authorize The Logo to secure medical treatment deemed necessary and assume all financial responsibility for such care.
7. Minors (Under 18) If the participant is under 18, this agreement must be signed by a parent or legal guardian. The undersigned parent/guardian acknowledges and accepts all terms on behalf of the minor, and consents to their participation. 8. Trainers & Independent Coaches (if applicable) Independent trainers or coaches using the facility acknowledge they are not employees of The Logo and must carry their own liability insurance. 9. Governing Law & Severability This agreement shall be governed by the laws of the State of Florida. If any portion is held invalid, the remainder shall remain in full force and effect.
