Inspire Nevada LLC 401 Vine Street Reno Nevada 89503 ACKNOWLEDGEMENT OF RISK AND WAIVER OF LIABILITY SPORTS PARTICIPATION CAN BE DANGEROUS. I recognize that participation in activities such as Cheerleading, gymnastics, tumbling, trampoline and youth Fitness can result in severe injuries, including, but not limited to, death, serious neck and spinal injury, paralysis, brain damage and other serious injuries that may result in the serious impairment of future ability for myself, child and family to earn a living, engage in business, and generally enjoy life. I am also aware that participation in some activities such as day camp and competitions involves transportation to and from field trips, which carries the risk of injury or death by vehicular accident. ON BEHALF ON MYSELF AND MY CHILD(REN), I ACCEPT ALL SUCH RISKS AND PROMISE NOT TO SUE, AND FOREVER RELEASE, Inspire Nevada LLC and its respective officers, directors, shareholders, employees, contractors, invitees, licensees and agents ("YOU") from all liability for damages and injuries incurred as a result of participation by my child(ren) or myself. This includes those injuries resulting from acts of negligence by you. I further acknowledge, understand, appreciate and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others and assume full responsibility for my participation and exposure. In the event of an accident or emergency I AUTHORIZE MY CHILD(REN) TO BE TRANSPORTED TO A MEDICAL FACILITY FOR TREATMENT, at my cost, and will hold you harmless in your management of such accident or emergency. I agree to provide for all medical expenses which may be incurred by myself or my child(ren) as a result of any injury while on your premises or while under your care. I KNOW ONLY STUDENTS ENROLLED IN CLASSES AND/OR REGISTERED IN AN EVENT ARE ALLOWED ON THE EQUIPMENT WITH STAFF SUPERVISION. I am aware that photos and videos are taken from time to time for marketing and instructional purposes, and I hereby consent to their use by you I wish to enroll myself and/or my child(ren) in Inspire Nevada, LLC. classes. Or will be inside Inspire Nevada Property for an event, mat rental with another team, party or other reason. I agree to follow any instructions or rules established by Inspire Nevada, LLC with regard to my and/or my child(ren)'s activities, whether written or orally given by Inspire Nevada, LLC personnel, including but not limited to the Rules and Regulations. I understand and agree that at any time, Inspire Nevada, LLC reserves the right to require me to remove my child(ren) from any activity for any reason and/or to remove my child(ren) from any activity for any reason and/or to cancel my and/or my child(ren)'s enrollment for any reason. I understand that every child is in a unique developmental stage and that I am most familiar with my child(ren)'s capabilities and limitations. As such, I agree to discuss any concerns with a staff member of Inspire Nevada, LLC that I have about my child(ren) which I think may affect my child(ren)'s ability to safely participate in any activities or to use any equipment. I am not aware of any physical or other condition which may affect me and/or my child(ren)'s ability to participate in any activity at Inspire Nevada, LLC. I am aware that if I and/or my child(ren) suffers from physical or other conditions which may affect their involvement at Inspire Nevada, LLC I agree to inform Inspire Nevada, LLC in writing by either email inspirenevada@gmail.com or during online Registration. 6. I agree not to hold Inspire Nevada, LLC responsible for any injuries suffered by me and/or my child(ren) while involved at activities at Inspire Nevada, LLC or that of its virtual class partners, or while in the lobby waiting for my children during their classes. I understand that if I designate a caregiver to be with my child(ren), that the caregiver will be required to sign a release and waiver prior to either the caregiver or the child(ren) participating in Inspire Nevada, LLC activities. I acknowledge, understand, appreciate, and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others and assume full responsibility for my participation and exposure. I agree to RELEASE, DISCHARGE, INDEMNIFY, PROMISE NOT TO SUE AND TO SAVE AND HOLD HARMLESS Inspire Nevada, LLC, its owners, officers, directors, contractors and employees, from any loss, liability, damage, or costs whatsoever arising out of or related to any loss, damage, or injury (including death) to me or my child(ren) arising out of or in any way connected with participation in the activities of Inspire Nevada, LLC for any reason or cause. In consideration for Inspire Nevada, LLC, permitting me and/or my minor child (“Child”) to voluntarily participate in all group fitness classes, any/all classes, parties, open gym, parents night out and all other associated classes, or events, to include the use of the all equipment and associated facilities located in Inspire Nevada LLC locations. I have agreed to execute this Acknowledgement of Risk, Release of Liability, and Appearance Release Agreement for myself and for my Child. I am 18 years old or older, and I and/or my Child, voluntarily participate in activities at Inspire Nevada, LLC. My child and I are in good physical health and condition. I am voluntarily using and/or allowing my child to participate in Inspire Nevada, LLC classes and equipment, with full knowledge of the inherent risks, hazards, costs, and dangers involved and hereby assume and accept all risks of injury, paralysis, or death. I, For Myself And My Child (And For All Of Our Heirs, Successors, Executors, Personal Representatives, Administrators, And Subrogors), Hereby Knowingly And Intentionally Fully And Forever Waive And Release And Hold Harmless Inspire Nevada, LLc, Including Their Members, Managers, Officers, Agents, Heirs, Successors, Trustees, Assigns, Executors, Personal Representatives, Administrators, Subrogors, Or Employees, From And Against Any And All Claims, Actions, Causes Of Action, Liabilities, Suits, Or Expenses (Including Reasonable Attorney’s Fees), Whether Foreseen Or Unforeseen, (A) Arising Directly Or Indirectly Out Of Any Damage, Loss, Injury, Paralysis, Or Death To Me, My Child, Or My Property As A Result Of Use Of Inspire Nevada’s Fitness Studio Or Equipment, Whether Such Damage, Loss, Injury, Paralysis, Or Death Results From Negligence Or Alleged Negligence Of Inspire Nevada, LLC, 9. I understand and agree that this release and waiver of liability, assumption of risk, and hold harmless agreement is governed by the laws of the State of Nevada, and is intended to be as broad and inclusive as is permitted by such law, and that, in the event any portion of this agreement is determined to be invalid, illegal, or unenforceable, the validity, legality, and enforceability of the balance of the agreement shall not be affected or impaired in any way, and shall continue in full legal force and effect. This release and waiver of liability shall be enforced and interpreted only by binding arbitration in Washoe County. I HAVE READ THIS DOCUMENT AND AGREE TO ALL OF ITS TERMS. I UNDERSTAND IT IS A LEGALLY BINDING AGREEMENT AND WAIVES CERTAIN LEGAL RIGHTS OF MINE, INCLUDING, BUT NOT LIMITED TO A RELEASE, WAIVER, PROMISE NOT TO SUE AND A HOLD HARMLESS FOR ALL CLAIMS. THIS AGREEMENT SHALL BE BINDING UPON MYSELF, MY CHILD(REN), AND OUR ESTATE, SUCCESSORS AND ASSIGNS. Rules & Regulations The following Rules and Regulations are designed to help ensure that you and your child(ren) have a safe and enjoyable experience at Inspire Nevada, LLC 1. Have fun! 2. I understand these documents and the entire agreement applies to me and all my children including those who may enroll in the future. 3. I understand that my child(ren) should participate barefoot for safety in all our gymnastics and lower-level tumbling classes and to maximize their gross motor development. If your child is enrolled in cheer shoes should be worn however shoes must be only used indoors on our gym floor. Socks are OK for adults in parent-child classes. During Fitness Club Classes all students will be required to wear shoes unless otherwise noted. 4. No food or drink (except water in a sealed, leak-proof container) allowed in the gym. 5. Although I understand that Inspire Nevada, LLC has attempted to create an injury free play area for me and my child(ren), a child needs constant attention and I agree to be either personally responsible for providing that attention or appoint another caregiver to provide that attention. 6. I understand that Inspire Nevada, LLC may compile and use photographs and/or videos of my child(ren) and me. I consent to such use and hereby waive all rights to compensation. 7. Do not bring your child(ren) if they or you are sick or injured. 8. I understand that I will not allow siblings or other children under my care who are not enrolled in a class to participate in that class, and that I am responsible for ensuring they wait quietly and not disrupt, participate or in any way create an unwelcome or unsafe environment at Inspire Nevada, LLC. 9. I understand that if I designate a caregiver to be with my child, that the caregiver will be required to sign a similar release and waiver before either they or my child may be allowed to participate in class. 10. I understand that Inspire Nevada, LLC will not be held responsible for lost, damaged or stolen property. 11. I understand that if I or my child(ren) damages any equipment or property while at Inspire Nevada, LLC not on accident, I will be responsible for paying any necessary repair or replacement costs. 12. We cannot refund fees if you or your child are absent as these payments hold your space at Inspire Nevada, LLC. 13. We do offer 1 makeup class per month this must be scheduled in our family portal (or using our App once available) within 24hrs of missed class. 14. I understand that proper attire is required in class for comfort and safety reasons. Your child should wear comfortable, easy to move in clothing that is not restrictive, heavy or bulky. Avoid nylon apparel (slippery). NO SOCKS • NO: jewelry, gum, jeans, skirts/dresses, exposed buttons, zippers, electronics. No loose clothing or loose anything. • HAIR: pulled back off of face; secure ponytails close to the neck (allows child to roll comfortably on floor) not high up on head. Remember your child will roll & tumble on mats & floor...avoid bulky hair clips or decorations that may cause discomfort during tumbling. 15. Students should remain at the gym when waiting for their ride home. For their safety, please do not have your child wait for you. 16. Inspire Nevada, LLC is not responsible for students left alone after their class has finished at the gym unattended. 17. Students may have their temperature checked prior to entering the gym. If a temperature is 100.4 degrees F or above, the student will be asked to sit and retake his/her temperature again. Students will not be allowed to participate if their temperature is 100.4 degrees F or above. 18. All participants could be asked to use hand sanitizer before entering the facility or to wash hands with soap and water prior to entering the gym floor. 19. Participants should bring a personal water bottle. We have a filling station on site if needed but no water fountain or cups available. In the event I or my child(ren) becomes injured or sick while participating in Inspire Nevada activities, I hereby consent to Inspire Nevada staff providing First Aid as well as summoning medical professionals to administer First Aid or emergency medical treatment for me and/or my child(ren). Release of Liability I, the minor's parent and/or legal guardian, understand the nature of these activities and the minor's experience and capabilities and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activity. I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability, claims, demands, losses, or damages on the minor's account, including negligent rescue operations, I further agree that is, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees (management, employees, contractors or staff of Inspire Nevada) I will indemnify, save, and hold harmless each of the releasees from any litigation expensed, attorney fees, loss liability, damage, or cost any may incur as the result of such claim. For Enrolled Athletes: Payment Policies Payment Information. I agree to provide a valid checking account or credit card information on file for automatic payment. Tuition is auto-charged MONTHLY on/near the 25th of the month for next month. Tuition is a flat monthly charge. Holidays and number of classes per month variables have been factored into the amount. If 5 classes in a month, we do not increase tuition and if 3 classes in a month we do not reduce. Pro-ration applied when enrollment changes mid-month (start/stop). I understand that monthly tuition is due the 25th of the month prior, regardless of whether or not we are able to attend, for that month and if I am late, including checks returned unpaid, that I will be charged a $35 late fee and may forfeit my spot in class to someone else waiting to enroll. Fees may be postmarked by the due date to avoid late fees. Tuition is considered 'on time' if post-marked by the due date. I agree to pay monthly tuition by the 25th of the month prior to enrollment and understand if it is not paid, Inspire Nevada will initiate electronic payment for my balance due onto my checking account or credit card which I am required by Inspire Nevada to have on file. I further understand that if tuition is not paid by the 25th of the month prior, I and/or my child(ren) will be listed as inactive, after five days of continued non payment my child will be un-enrolled, and the spot(s) will be released to students waiting to enroll. I understand that Inspire Nevada does not bill me for tuition nor wait for payment. It is my responsibility to have funds available in my account to pay my family balance via auto-pay. I understand that I and/or my child(ren) are continuously enrolled in class and will incur tuition charges on my account every month until I inform Inspire Nevada, in writing, 30-days in advance of my child's last class. Drop Policy:
Email Written notice is required to discontinue classes. Requests must be submitted by the 5th of the month prior to your final month of enrollment. This allows time for billing adjustments, staffing, and opening waitlist spots. Families with another actively enrolled child may qualify for a reduced 15-day notice. The online registration process is not a guaranteed spot in class until Inspire Nevada receives payment and you are confirmed. We cannot refund fees if you or your child are absent as these payments hold your space. I agree to pay a $45 fee for any check that is returned unpaid by my bank or financial institution. I agree to provide written notice 30-days in advance of the last class we plan to attend if I wish to withdraw from Inspire Nevada. CLASS PAYMENT OPTIONS CASH/BANK DEBIT/VISA/ MASTERCARD/DISCOVER. I agree to pay $40 for any valid credit card charge that I dispute with my credit card company. I further understand that if my credit card is declined there will be a $20.00 decline fee charged. Electronic Checks and Credit Card payments may be initiated online through your personal portal account on our website www.inspirenevada.com Enrollment Defined Upon enrollment your space is reserved in the class(s). Enrollment is continuous and must be cancelled when you want to withdraw. Tuition reserves your space in the class(s); it does not guarantee a certain number of classes. Written Notice to Withdraw Written notice to withdraw from classes is due before the 25th of your last month. Notice is required each time your child decides to take a break from classes. If no withdrawal notice is given, your debit card, credit card or bank account on file will be charged automatically for the next month's tuition. Students may be automatically withdrawn from class for the next month if tuition is not received by the last day of the month prior to classes starting. All class drop requests must be submitted no later than the 25th of the month prior to your final month of classes. Example:
If you plan to discontinue classes for April, your drop request must be submitted by February 25th. This policy allows us to: - Properly plan and staff classes based on enrollment
- Process our automatic billing system accurately
- Open spots in full classes for families currently on our waitlist

If you have another child who will remain actively enrolled in classes, we are happy to offer a reduced 15-day notice for your drop request. All drop requests must be submitted in writing via email to ensure proper processing. Email Written Notice: farrah@inspirenevada.com Photo Release I authorize Inspire Nevada to use any photographs that contain images of myself or any of my children for any purpose for furtherance of the business of Inspire Nevada, including, but not limited to, inclusion of any such images in advertisements, on websites, or on posters. I further understand that no personal information will be associated with any such images, including, but not limited to, name, age, address, school affiliation or any other identifying information, without my prior written consent. Inspire Nevada may use pictures on social media and other advertising methods, however we will make you aware if your child’s picture or video is used in any other major advertising marketing campaigns. Holiday Closures 2026 Scheduled Gym Closures: · Thanksgiving November 25th - 29th · Winter Break December 23rd - January 3rd 2027 Scheduled Gym Closures: · Memorial Weekend May 28th - May 31st · Week of July 4th July 3rd - July 11th · Labor Day Weekend Sept 4th - Sept 6th · Thanksgiving November 24th - 28th · Winter Break December 23rd - January 2rd Year Round Program We are a year round program with a monthly tuition based on an average of four classes per month. If there are five classes in a month, we do not increase tuition. If there are three, we do not credit. Make-up Policy You must schedule your makeup class through our family portal within 24hrs following a missed class. Makeups must be scheduled within 1 week of the missed class through the family portal.
