Gym Attestation 

2026-2027

The undersigned, as primary owner and operator of the gym named below or as the principal of a participating school (hereinafter referred to collectively as the “Organization(s)”), acknowledges and attests to “Canadian Cheer” (comprised of 7666705 Canada Inc., 6763201 Canada Inc., and 10892262 Canada Inc. and each of their respective directors, officers, employees, sub-contractors, agents, representatives, affiliates, subsidiaries, related companies, successors, and assigns) that the undersigned has conducted a Vulnerable Sector Check on each of the coaches or representatives of the Organization (hereinafter referred to collectively as the “Coach(es)”), who will be participating in any of the applicable 2026-2027 Canadian Cheer event (the “Canadian Cheer Event”).

The undersigned hereby confirms that such Vulnerable Sector Check does not reveal: (i) any criminal activity, (ii) any ineligibility of a Coach to attend and participate in the Canadian Cheer Event, (iii) anything that would preclude a Coach from being in the physical presence of and in close proximity to minors, or (iv) anything else that could be of concern and to an event convenor and operator.

The undersigned further attests that all persons listed on the submitted roster of Coaches for the Canadian Cheer Event are not currently under suspension by Cheer Canada, the recognized National Sports Organization (NSO) for cheerleading in Canada, or by any specific Provincial or Territorial Sports Organization (PTSO) for any reason.

The undersigned agrees, on behalf of the undersigned and the Organization, to defend, indemnify and hold Canadian Cheer harmless from any and all claims, demands, actions and proceedings related to any claim made against Canadian Cheer, including for alleged negligence, wilful misconduct or misrepresentation, which relates to or is based upon any of the information represented and attested to herein. By signing this attestation, the undersigned acknowledges that Canadian Cheer is relying on this attestation in allowing the Organization’s Coaches to attend and participate in the Canadian Cheer Event.

I hereby acknowledge that I have the authority to bind the organization and agree to the above attestation relating to the coaches listed on our coach’s roster for the Canadian Cheer event.
