1. ASSUMPTION OF RISK & NATURE OF SERVICES

The Woojer Acoustic Haptic Mat delivers powerful, low-frequency tactile vibrations and acoustic sound waves directly to the body. This haptic technology is designed to enhance mindfulness, music immersion, nervous system down-regulation, and acoustic relaxation post-workout.

I understand and acknowledge that:

  • Woojer technology utilizes strong tactile transducers that transmit physical kinetic vibrations and low-frequency bass energy directly through bones, tissues, and muscles.
  • The physical effects may include deep muscular vibration, sensory stimulation, mild temporary lightheadedness, or auditory stimulation.
  • Haptic acoustic mat services provided by Be Awesome Be Strong LLC are intended solely for general relaxation, sound therapy, and athletic recovery, and do not constitute medical treatment, physical therapy, or medical rehabilitation.


2. CONTRAINDICATIONS & HEALTH ACKNOWLEDGMENT

Because the Woojer Acoustic Mat generates intense physical vibrations, magnetic fields, and low-frequency tactile energy, it presents specific safety risks for individuals with certain medical implants or physical conditions.

By signing below, I certify that I DO NOT have any of the following conditions, OR that I have received explicit written clearance from a licensed healthcare provider to use acoustic/haptic vibration equipment:

  • Pacemakers & Implanted Medical Devices: I do not have a cardiac pacemaker, implanted cardioverter-defibrillator (ICD), insulin pump, or other active electronic medical implant that could be affected by magnetic components or vibration frequencies.
  • Pregnancy: I am not currently pregnant or suspect I may be pregnant (direct abdominal/back vibration is contraindicated during pregnancy).
  • Epilepsy / Seizure Disorders: I am not prone to sound- or vibration-induced seizures or light/sensory-triggered neurological events.
  • Recent Physical Trauma & Surgeries: I do not have acute unhealed bone fractures, joint replacements, recent surgical incisions, or acute spinal injuries (e.g., severe herniated discs) that could be aggravated by mechanical vibration.
  • Severe Cardiac Conditions: I do not have unmanaged arrhythmia, severe heart disease, or acute cardiovascular instability.
  • Inner Ear / Balance Disorders: I am not prone to severe motion sickness, vertigo, or Meniere's disease triggered by low-frequency acoustic vibrations.

If you checked or suspect any of the conditions above, you must inform Be Awesome Be Strong LLC staff prior to your session.

3. CLIENT RESPONSIBILITIES & RULES OF USE

I agree to strictly adhere to all operational guidelines provided by Be Awesome Be Strong LLC staff:

  1. Intensity Levels: I will start sessions at a low-to-moderate haptic intensity level and will not increase the vibration or volume settings beyond a comfortable, soothing threshold.
  2. Immediate Stop: If I experience dizziness, nausea, chest pain, numbness, headache, or physical discomfort at any point during the session, I will stop the device immediately and notify staff.
  3. Headphones & Auditory Safety: I will keep headphone audio volumes at a safe, moderate listening level to protect my hearing.


4. WAIVER AND RELEASE OF LIABILITY

In consideration of being permitted to use the Woojer Acoustic Haptic Mat and recovery services at Be Awesome Be Strong LLC, I, for myself, my heirs, personal representatives, and assigns, do hereby:

  1. RELEASE, WAIVE, AND FOREVER DISCHARGE Be Awesome Be Strong LLC, its owners (including Beth Anne Moonstone), operators, officers, employees, coaches, independent contractors, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including neurological discomfort, joint/spinal aggravation, hearing issues, or personal injury) that may be sustained by me while using the Woojer Acoustic Mat equipment.
  2. COVENANT NOT TO SUE Be Awesome Be Strong LLC for any claims arising from the normal and intended use, or accidental misuse, of the acoustic haptic equipment.
  3. INDEMNIFY AND HOLD HARMLESS Be Awesome Be Strong LLC from any loss, liability, damage, or costs (including legal fees) that may incur due to my participation in these services, whether caused by negligence or otherwise, to the fullest extent permitted by Massachusetts law.


5. ACKNOWLEDGMENT OF UNDERSTANDING

I have read this Informed Consent & Liability Waiver carefully and fully understand its terms. I understand that I am giving up substantial legal rights, including the right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
